(More) This is a 2010 response to a 2006 FOIA request to the US DHHS Office of Inspector General for the recertification applictions from the Illinois Medicaid Fraud Control Unit house in the Illinois State Police Department. This application states that the IL AG has NO JURISDICTION OR AUTHORITY to prosecute Medicaid Fraud. Only the County State's Attorney or U.S. Attorney may prosecute such crimes. IL AG Lisa Madigan has, without knowledge or assistance of the Cook County State's Attorney or U.S. Attorney illegally indicted and prosecuted at least four people including Linda Shelton (not guilty at jury trial 2/2009); Vernon Glass (deceased before trial so charges dismissed); Maisha Hamilton (fraudulent trial held that is void due to criminal conduct of AG Madigan in obtaining fraudulent indictment, no arrest having actually occurred, and therefore no jurisdiction of the court, Judge Schultz, who claims to have convicted her and Judge Gainer who resentenced her without jurisdiction as the case is in the Illinois Appellate Court to have it declared void); and Naomi Jennings who was fraudulently convicted with a invalid indictment, by a prosecutor and court without jurisdiction.

Since the IL AG had no jurisdiction and the cases were void or a nullity, the State through the Illinois State Police Director Nolan and IL AG Madigan have fraudulently obtained millions from the federal governmnet to use to investigate and prosecute these people on fraudulent charges. State Police Investigator William Reibel fabricated false invoices to use against Maisha Hamilton by cutting out handwriting exemplars she was forced to sign and pasting them on phony invoices. The IL State Police Crime lab refused to study them as Reibel sent them only faxes. Hamilton was allegedly convicted in 2003, but the trial was void and a real sham. Nothing was legal the way it was done and the fact is that Hanilton was never even legally arrested, therefore the conviction is void. I viewed xerox copies of these phony invoices and handwriting exemplars myself and can verify that it is easy to match the signatures on the invoices with specific signatures on the handwriting exemplars. Any handwriting expert will be able to study and testify about this.

As to my case a couple of women conned the CEO of a psychiatry group that I was going to work for into letting them run a branch office. I gave the CEO my doctor's Medicaid number the year before I joined the group so the billing agent could sign me up while I was undergoing major surgery and recovering in preparation for me working there. The billing agent instead forged m name on Power of Attorney and Alternate payee forms, which you can see here on page 5 and 6 and then when these two women submitted fraudulent bills to her using my name, the billing agent, Louise Moore of Data MedicalWorks sent in the bills under my Medicaid provider number. This occurred before I even began working for the group and without the knowledge of the CEO Vernon Glass.

I received the forged documents from the IL AG in discovery before trial and so I know they are fully aware that this was a case of ID theft and I was innocent. So they knew there was no probable cause and they continued the prosecution from illegal indictment in 2004, which AG Madigan obtained without jurisdiction, until the jury found me not guilty in February 2009.

The only reason AG Madigan prosecuted me was for harassment and to discredit me. She has also wrongfully, in acts of defamation spread the word that I am mentally ill. This is entirely false. I will post mental health records revealing I have been examined and found to be mentally intact, with NO evidence of ever suffering from delusions, hallucinations, or mental illness except for post-traumatic-stress disorder after being sexually molested as a teen and after the memories were rekindled when I was battered and assaulted by a police officer.
