Medical Group Management Association's 51 page booklet entitled "The Superbill, Guide to a Uniform Billing and/or Claims System" (IsBN 0685033295 -- published 7-01-1974) represents the culmination of a 4 year effort that started in Minnesota in 1970. The concept of a standard billing/insurance system for patient services initiated by physicians (rather than the insurance industry) was an idea defined and named by me (Bruce McKinnon) and John Strehlow, Minnesota medical practice administrators during the early 1970's. During 1972, John and I both left Minnesota to take new positions in North Dakota and Mississippi, respectively. However, we were both appointed to Medical Group Management Association's (national office in Denver) Procedures Committee as the Superbill concept moved into the national spotlight. During the years that followed, I wrote two major articles in the Journal of the MGMA (1972 and 1973) first defining the Supebill concept and then addressing feedback to the first article. To help achieve a consensus prior to publication of this GUIDE, formal Superbill presentations / discussions sessions were conducted at two annual MGMA national association conventions. During one annual event, a long line of bulletin board displays containing thousands of check-off service tickets sent in by medical groups from throughout the US (representing every medical and surgical specialty) literally circled a large exposition hall packed with vendors -- and proved to be a major hit. These "service tickets" later became an important component of the Suberbill system (see see pages 39 to 42 in this booklet). It is also interesting to note that in the years that followed this publication, various medical practices often referred to the service ticket (mistakenly) as the Superbill. By 1974, The Superbill was a thoroughly defined concept. Now, MGMA members focused on their desire to see the actual form (the Superbill). I spent from January to June 1974 designing the Superbill System 3 part form with a representative from the Moore Business Forms, Mobile, Alabama office. The objective was to be able to use the format with a pegboard system, a mark sensing device, computer or the lay-out burned into a computer generated tape to tape format. Once completed and edited, I laid out and wrote the instructions for the Superbill, incorporating the 3 page format into the 51 page GUIDE presented here. All drafts of the final document were reviewed by the members of the MGMA Procedures Committee. However, the lion's share of the editing was done by John Strehlow and Stan Hager (Everett Clinic, Everett, WA) who had written a land mark 1969 article on the use of check-off service tickets in a multi-specialty environment. The forward to the Superbill publication was written by Bill Costello (Administrator of the Nicollet Clinic, a large multi-specialty group practice in Minneapolis, MN), past President of Minnesota MGMA and the 1973-1974 President of Medical Group Management Association (Denver). The "History" leading up to the Superbill was written by John Strehlow, Administrator of the North Dakota School of Medicine. The 1974 MGMA publication was distributed to Medical Practice Administrators, IT staff, and service bureaus throughout the US. The billing coding systems (CPT & ICDA-9) became the billing standard. Service tickets (often referred to as the Superbill) became the standard used to capture patient encounter services and diagnosis. Both are used by physicians groups to this day (2010). All of the Superbill components (service tickets, multi-mode billing systems from peg boards to computers and tape to tape downloads to the insurance industry) became standards. The AMA standard health insurance claim form continued to evolve spurred by insurance industry-wide coverage for out-patient services that followed Medicare's lead for coverage of out-patient services and preventative health care. Introduction to my article entitled "The Super Bill" that appeared in the November/December 1972 issue of Medal Group Management, Journal of the MGMA (Vol 20, #1, pp 21-26). While a Medical practice Administrator in Minnesota, member of Medical Group Management Association and as Chairman of Minnesota Medical Group Management Association's "Clinic-Third Party Billing Procedures Committee", I had become frustrated with a growing proliferation of insurance company forms and promoted the idea of a physician generated standardized billing system for physician services that, as a by-product, would also serve the needs of the Insurance industry including Medicare and Medicaid. With the help of another medical practice administrator and Committee member, John Stehlow, we envisioned and portrayed system that would implement a nationwide coding system for medical, surgical & ancillary services and diagnosis. The concept was revolutionary at the time because it would be developed and implemented by the physician side of the health care industry rather than by the insurance industry which had become bogged down in their attempts to develop a standardized insurance claim form that would satisfy that industry. Given my background as a Medicare Part B Unit Supervisor (Travelers), it was apparent that this new concept address both out-patient services (not then covered by private insurance company contracts) and in-patient services. Following a two year (1970-1971) effort to promote this concept in Minnesota (which John and I labeled as the Superbill, a name conceived prior to a Super Bowl game), it was apparent that it would have to cross state lines to be successful. John and I were appointed to the MGMA Clinic Procedures Committee. This article provides a detailed description of the Superbill concept and the two year road that led to the publication. BWM
