GeneralInstructionsPurpose.TheAnnualProgressReport (APR)isareportingtool that HLIDuseso track programprogressandaccomplishmentsnd informthe Department'scompetitive processor homelessassistanceunding.FilingRequirements.Recipientsf HUD'shomelessssistancerantsmustsubmit2 APR'StoI{UDwithin90 days aftertheend ofeachoperatingYear.Onecopyof the reportmust be submittedothe Communityplanninguoan.*top-.ot(CPD)DivisionDirectorin the localHUDFieldOffice responsibleormanaginghe grant.Theothercopy mustbe submittedtoHLIDHeadquarters,Deparlmentof HousingandUrbanDevelopment,Attn:APRDataEditor,BoomjieZ,4517h Sheet,SW,Washington,DC.20410.Failuretosubmitan APRwill delay receivinggrantfundsand mayresultina determinationoflackof capacityorfuturefunding.An APRmustbesubmitted oreachoperating yearin whichHIIDfunding is provided.Granteeshat receivedSHPfundingfor newconstruction,acquisition, orrehabilitationare requiredtooperate heirfacilitiesfor20yeats.Theymustsubmitan APR90 daysafterthe endofthe firstoperating yearandeveryyearthroughouthe 20 years.Aseparateeportmustbe submittedoreach HUD grantreceived.ForShelter PlusCare(S+C),aseparateApRmust besubmittedfor eachS+C component.Forthose granteeseceivingan extension,a separateepoftcovering that periodmustbe submitted(seeExtensionbelow).Recordkeeping.Granteesmustcollectandmaintaininformationoneachparlicipantinorder tocompleteanApR.Optionalworksheetsareattached.The worksheetsmaybeused orecord informationmanuallyorto designaiomputerizedsystem ostore andtabulateheinformahon.Theworksheetsshouldnot be submittedo HUDwiththe AFR.organizationofthe Report.TheAPR isorganizednthe followingmanner:PartI:ProjectProgress.This portionofthe reporl describesheprogressn movinghomeless ersonso self-sufficiency,documentingserviceseceived,isting project goals,andaccountingor beds/units.Part II:FinancialInformation.This portionof the reportis completedbyallgranteeseceiving{irndingunderSHp,S+C,andSRO.FinalAssemblyof Report.Afterthe entirereport isassembled, urhbereverypagesequentially.Markanyquestionshat donotapplytoyourprogramwith"N/A" fornotapplicable. (SeeSpecial nstructionsforSSO Projectsbelow.)DefinitionsofClient/IlouseholdTypes.Eachclientihouseholdype isdefinedbelow.Notethata client'sclient/householdtypeshouldbe basedonthe client'sage and/orhouseholdcomposition attheprogramenttydateclosesto thestart oftheoperating year.tr'amilies-A familyis a householdcomposedof fwoormorerelatedpersons,at leastone ofwho isa child accompaniedby an adultor auvenileparent.Singlesnot inFamilies*Personsnotaccompaniedby children, includingpregnantwomennotaccompaniedbyotherchildrenandunaccompanied outh,aresinglesnotin families. Whentwo adultsor twounaccompanied outhpresenttogether orservices,eachpersonshouldbe countedn singlesnotin families..Clients'householdstatusshouldbedeterminedbasedon their householdcompositionattheprogramentrydateclosesto thestartof theoperatingyear.Thismeanshat pregnantwomenexpectedogivebirth during their programstay shouldstillbe countedassinglesnotrnfamilies.AdultsinFamilies- Withina family,an adultis any person18yearsof age orolder. Forthe purposesof APR reporting,the determinationof whetherapersonsan adult in familyshouidbe madebasedon their ageand householdcompositionat the programentry dateclosesto the startofthe operatingyear.Children in tr'amiliesChildren n Familiesare dehnedas childrenunder he ageof 18accompaniedy oneor moreadults(parent,relativeorguardian).Children in familiesalso nciudebothauvenileparentand the parent'schild(ren).Forthepurposesof APR reporting,thedeterminationof whetherapersonsa child infamilyshouldbe madebasedontheirage and householdcompositionat theprogramentry date closesto the startof the operating year.Forexample,

clientswhoareess han18years of ageonthe firstdayof theoperatingyearor atprogram entry(iftheyenteredduringthe operatingyear)shouldbe countedas childreneven ftheyturn18 duringthe courseofthe operatingyear.PersonsinFamilies-Persons n familiesincludes adultsnfamiliesandchildren in families.Other Key Definitions. The following tems areused n the APR.As indicated, in somecases,ermsare applied differentlydependingon whether he funding is from SHP, S+C,orSRO.Chronically homelesserson- HUDdefinesa chronicallyhomelessersonas"anunaccompanied omelessndividualwitha disabling condition who has eitherbeen continuouslyhomelessor ayearor moreOR has hadatleastour(4)episodes f homelessnessnthepasthree3) years."To be considered hronicallyhomeless,personmust havebeen onthe streetsor in anemergencyshelter(i.e.,not in transitionalhousing)during thesestays.HUD'sdefinition of a chronicallyhomelessersonsbasedon the followingcomponents:oUnaccompaniedhomelessndividual: an unaccompanied omelessndividual hashe same haracteristicsof a Singlenot in a Family(describedbove)..Disabling condition:see heinstructionsunderdisablingcondition(below)to determinewhetheraclient isdisabled.Didnotleavetheprogram-Thistermreferstoclients who were in the programon the lastday of the operating year.Disabling condition-HUD definesa disablingconditionas: 1)A disabilityas defined n Section223 of theSocialSecurityAct; (2)aphysical, mental,or emotionalimpairment which is (a)expectedo be of long-continuedand ndefiniteduration,(b)substantially mpedes an individual's ability toliveindependently,and(c)of sucha nature hatsuch abilitycould beimprovedbymoresuitablehousingconditions;(3)adevelopmentaldisabilityas dehnedn section 102of theDevelopmentalDisabilities Assistanceand Bill ofRights Act;(4)the diseaseofacquired mmunodeficiencysyndrome orany conditions arising from the etiological agency oracquiredmmunodeficiencyslmdrome;or(5)a diagnosablesubstance busedisorder.Entered theprogram -Entered heprogramrefers to thefirstday a client receives services.For a residentialprograrn,this date would represent he first day of residence n theprogram'shousing. For services,his date may representhe dayofprogramenrollment, the day a service wasprovided,orthe firstdate of aperiodof continuous participationina service(e.g.,daily, weekly, or monthly).For S+C and SROprograms,heprogramentry date s the date that theparlicipantstartsoreceiverentalassistance. ForS*C, servicesprovided priorto thispointare recognized as necessary or outreach./enrollmentndare eligibieto count asmatch.An Extension APR applies o SHP andS+Cgranteeshatrequestedand receivedan extensionof theirgrantterm fromthe HIID field office. The only difference between an APRfor theextensionperiodand the regularAPR(besidesheamount of time covered)s the signaturepage.Granteesshould circle "yes"toindicatethe APR is foran extensionperiodand circle the operatingyearfor which the report is an extension.For example, fthegranteesextendingyear3,thegranteeshould submit anAPR asusualforyear3 andsubmit another APR for theextensionperiod,indicatingtheseconds an extensionand also circlingyear3 onthesignaturepage.Grantee meansa direct recipient of the HIID award.Lefttheprogram- Left theprogramefers o the ast day a client receiveservices.For a residential rograrrlhis datewould represent he last day ofresidence ntheprogram'shousing. Forservices, he exit datemay representhelastdaya servicewasprovidedorthe ast dateofaperiodofcontinuousservice. Ifaclient eaves heprogramtemporarilye.g.,fora hospitalization)but is expectedoreturn within30days, do not count thatclient as having lefttheprogram.For S+Cprograms,heprogram exit date refers to the date heparticipantstops eceivingrental assistancend is notexpected o return to S+Cassistedhousing. Iftheparticipantreturns o S+C assistedhousingwithin90 days, he personshould not be consideredas exitrngfromtheprogram. If thepersonretums toS+C assistedhousingafter 90days, thatpersonsconsidereda newparticipant.The worksheet s designed o capturehis information.
