General InstructionsPurpose.The Annual ProgressReport(APR)is areporting oolthatHUDuses o lrackprogramprogress ndaccomplishmentsnd nfonnthe Department'scompetitiveprocessot'homeless ssistancetiirding.FilingRequirements.Recipients f HUD'shomelessssistancerantsmust submit2 APR'S to HUD n'ithin 90 davs altertheend of eachoperating vear.Onecopy of thereporl mustbesubnrittedotheCommunityPlanningndDeveloprnent(CPD)DivisionDirectornthe ocal HUDField Officeresponsibieor managinghegrant.Theothercopymustbesubrmttedto HIJD Headquarters,epartmentof Housingand UrbanDevelopment,ttn: APR Data Editor,Room 7261,451 7'hSfteet,SW,Washington,DC. 20410. Failure o submit anAPR rvi1ldelayreceiving rantundsandmay result na determination flack of capacity or future funding. The APR is organizedn the foliowingmaluler:Part I:ProjectProgress.Thisportionof the reportdescribesheprogressnmoving homelessersonso self-sufficiency,documentingerviceseceived, istingproject goals,and accountingorbeds/units.Part II: Financial Information. Thisportion of thereport s completed y allgranteeseceivingundingunderSHP,S+C,and SRO.FinalAssembly of Report. After the entire eports assembled,umbereverypagesequentially.Markanyquestionshatdonot appiy toyourprogram with"N/A"for not applicable.(SeeSpecialnstructions orSSOProjects elow.)Definitions of Client/Ilousehold Types. Each client4rouseholdypesdefinedbelorv. The following tem$ areusedntheAPR.Asindicated,n somecases,emrs are applieddifferentlydependingn rvhetherhe funding s fromSHP, S+C,orSRO.Chronically homeless erson- HLrD definesa clu'onicallyomeless ersonas"annnaccompaniedomeless ndividual,,vithadisablingcondition.vhoaseitherbeencontinuouslyomelessora yearor moreOR hashad atleast our(4)episodesfhomelessnessnthepasthree3) years."To be consideredhronically omeless, personmusthavebeenonthe streetsrinan emergency helteri.e.,not in fransitional ousing)during hese tays.HUD's definitionofa chronicallyhomelessersons basedonthe following components:.Unaccompaniedhomeless ndividual:an unaccompaniedomeiessndividualhas he samecharacteristicsofa Singlenot in aFamily(describedbove).rDisablingcondition:see he nstructions nderdisablingconditionbelow)o deterrninewhethera clientsdisabled.Did not leaveheprogram-Thistermrefelso clientswho were n theprogramon the astday of theoperatingear.Disablingcondition - HUD definesa disablingconditionas:1)A disabilifyasdefined nSection223oftheSocialSecurityAct;(2)aphysical,mental,or emotionalmpairmentwhich s(a)expectedo beof long-continuednd ndefiniteduration,b)substantiallympedes nndividual'sabiiity toliveindependently,nd(c)of sucha nature hatsuchabilitycouldbe improvedbymoresuitable ousingconditions;3)adevelopmentalisabilityasdefined n section102of theDevelopmentalDisabilitiesAssistancendBillofRightsAct;(4)the diseasef acquiredmmunodefrciencyyndlomeoranyconditionsarisingrom the etiologicalagencyor acquiredmmunodeficiencylmdrome;or(5)a diagnosablesubstancebusedisorder.Enteredtheprogram - Enteredheprogram efersothe first day a clienteceiveservices.Fora residentialrogram,thisdatewould representhe first dayofresidencen theprogram'shousing. Forservices, hisdatemay representhedayofprogramenrollment,he day a servicewasprovided,orthe frstdateof aperiodof continuous articipationna service(e.g..daily.weekly,rmonthly).For S+Cand SROprograms,heprogramentry date sthe date hat he participanttartso receiveentalassistance.orS*C, servicesrovidedpriortothis pointareecognizedasnecessaryor outreactvenrollment ndare eligible ocountasmatch.An ExtensionAPR applies o SHP and S+Cgranteeshatrequested ndeceivedanextensionof tlieirgrantermromthe HUDfield office.
