Whentwo adultsortwo unaccompaniedouthpresenttogether orservices,achpersonshouldbe counted nsingies ot infamiiies..Clients'householdtafushoul,Cedetermined basedon their householdcompositionat the programenfrydate closestothe startof theoperatingyear.Thismeanshatpregnantwomenexpectedogivebirth duringtheirprogramstay shouldstillbecountedas sinelesnotinfamilies.Adults in Families- Withina family,an adult sanyperson18yearsof ageorolder.For the purposesf ApRreporting,the determiaationofwhethera persons an adultin familyshould be madebased ontherageandhouseholdcompositionatthe programentry date closesto thestart ofthe operatingyear.Children in Families-Children n Familiesaredefiaedas childrenuaderhe ageof18accompaniedy oneor moreadults(parent,relativeor guardian).Children infamiliesalsoncludeboth auvenileparentandthe parent'schild(ren).For thepurposesof APRreporling,the determinationof whether apersons a childinfamilyshouldbemad.easedontheirageand householdcomposifionattheprogramenfrydate closesto the startof theoperating year.Forexample,clients whoare esshan 18years0fageon the frst dayofthe operatingyearoratprogramenfi-yiftheyenteredd*r.tgtheoperatingyear)shouldbe countedas childreneven fthey tum 18durrng he courseoftheoperatingyear.

Fersonsin Fainilies- Personsnfamrliesncludes adultsn famrliesand childrenrnfamthes.Other KeyDefinitions.Thefollowing terms are usedntheAPR.Asindicated, insome cases, erms areapplieddifferentlydependingon whether the funding isfromSHP, S+C,orSRO.Chronicailyhometresserson- HLID definesa chronicailyhomeless ersonas"anunacconrpaniedomelessndividualwith a disablingcondition u'ho has eitherbeenconthuously homelessorayearor more ORhashad at leastour(4)episodesf homelessnessl thepasthree3) years."To be consideredhronicallyhomeless,personmust havebeenonthe streets r inan emergency helteri.e.,not intransitional ousing)duringhese tays.HUD's definitionof acluonically homeless ersons basedonthe followingcomponents:Unaccompanied homelessndividual:an unaccompaniedomelessndividualhashesamecharacteristicsof aSrnglenot in a Family(describedbove).Disablingcondition: seehe nstructions nder disablingcondition below)todeterminewhetheraclient isdisabied.Did not leaveheprogram-This term referso clientswho weren the programon the astdayof theoperating ear.Disablingcondition-HIIDdefinesa disablingconditionas:1)Adisabilityas defiredinSection223of theSocialSecurity Act;(2)aphysical,mental, or emotional rmpairmentufuch is (a)expectedo be oflong-continuedandndefiniteduration, (b)substantiallympedes an individual'sability to live independently,and (c)ofsucha nahuethat suchabilitvcould be improvedby more suitable housingcondifions;(3)adevelopmentaldisabilityas defuredn section102oftheDevelopmentalDisabilitiesAssistanceand Billof Rights Act;(4)thediseaseof acquiredmmunodeficiencysyndromeorany conditionsarisingfromtheetiological agency Oracquiredmmunodeficiencysy.ndrome;r(5)adiagnosablesubstance busedisorder.Entered the program -Entered the programrefers to thefrstday a clientreceivesservices.Fora residential prograrqthisdatewouldrepresenthe first dayof residence n theprogram'shousing.For services,hisdatemayrepresenthe dayofprogramenrollment, theday a service wasprovided,or the fust dateof aperiodof continuousparticipationina service(e.g.,daily, weekly,or montlily).ForS+C and SRO programs,theprogramentry date sthedate thatthe participantstartso receiverentalassistance.ForS*C, servicesprovidedpriorto thispointare recognizedas necessaryor outreach/enrollmentandare elieibletocountasmatch.An ExtensionAPR applies to SHP andS+Cgranteeshat requestedand receivedan extensionof their granttermfromthe HLD freld office. The only differencebetweenan APRfor theextensionperiodand theregularAPR(besidesheamount of timecovered) is the signaturepage.Granteesshould circle"yes"to indicatethe APRisforan extensionpenodandcircle the operatingyearforwhich the report is an extension.For example,f thegrantees extendingyear3,thegranteeshouldsubmitanAPR as usual foryear3 and submit anotherAPR fortheextension period,indicatingthesecond s anextension and also circlingyear3on thesignaturepage.Granteemeansa direct recipientofth:HLD award.Left theprogram- Left theprogramrefers to the lastdaya clientreceivesservices. Fora residentialprograr4thisdatewould representhe last dayofresidencentheprogram'shousing.Forservices, heexitdatemayrepresenthe lastdaya servicewasprovidedor the ast dateof a periodof continuous ervice.Ifa client eaveshe programemporarily e.g.,forahospitaluanon)butis expectedo retumwithin 30 days, do notcount thatciient ashavingleft the program.For S+Cprograms,theprogramexit daterefers to the date heparticipantstops eceivingrentalassistancend snotexpectedo returnto S+Cassistedhousing.If the participantreturnso S+C assistedhousingwithin90 days, he personshould not be consideredas exithg fromtheprogram.If thepersonreturnstoS+Cassistedhousingafter90days, hatpersons considereda new parficipant.
