Dear Head Teacher, Leave Application for Absence on Exams My son, Daniel Tan Wei Jun (1092011060031) , is currently studying in PTPL College and he is attach to the course Diploma in Information Technology . Please be advised that I wish to humbly request for a leave of absence on the exams for my son . 2. Daniel Tan Wei Jun is only in his second semester for the above mention course , and the final semester exam will be held on the day that he will not be in Malaysia. My son together with the family will be going for a religious activity in Phuket , Thailand on 4th March 2012 . Me and my son will be leaving for Phuket, Thailand on 1 st March 2012 and will be back on 5th March 2012 . Attached is an itinerary of our trip for your further reference . This trip was plan last year to coincide with a family vacation . Regrettably , the subject (Pengenalan Kepada Pengaturcaraan KCS 1083) will be held on 2nd March 2012 , hence clashes with the date that he will not be around . Therefore , I would like to request for a leave of absence from exam on 2nd March 2012 for Daniel Tan Wei Jun . He is willing to sit for this exam on an earlier or later date depending on your convenience and regulations. 3. I understand that it is extremely difficult on your part to make changes to the exam date and could not underestimate the importance of the exams, but the trip and this religious activity are important for us as a family as well .
