Upon
release fromimprisonment,thedefendantshall
beon
supervisedrelease
for
a
term
of
THREE

years.
While
on
supervisedrelease,thedefendant
shallcomplywiththemandatory,
standard
andifapplicable,the special
conditionsthathave
beenadoptedby
this
Court,andshall complywith
thefollowingadditionalconditions:

defendant
shallnot useany
computer
at anylocation
without the priorwrittenpermissionofthe
probationofficer.Thedefendantshallnotpossess oruse
a
phoneorany other
electronicdevice thatallowsaccess
to theInternet
withoutpriorwrittenpermission
of
theprobation
officer.XThedefendant shall
participate
in
theComputerRestriction/Monitoring Program(CRMP)andshallabide
by all
rulesand
requirements
of
theprogram.Thedefendant
shallabstain
from
the useofalcoholand/or
all
otherintoxicantsduring the termofsupervision.XThedefendant
shall
submit
to
an
evaluationforsubstance abuse ordependencytreatment
as
directed
by
theprobationofficer, and ifdeemednecessary
by
theprobation officer,the
defendant
shallparticipate
ina
program
approved
by
theprobation
officer
for
treatment
of
narcotic addictionordrug
or
alcoholdependencywhich
may
includetesting
and
examination todetermine
ifthe
defendant
has
reverted to theuse
of
drugs oralcohol.Duringtreatment,thedefendantshallabstain fromtheuse ofalcohol
and
any
and all
intoxicants. Thedefendant may
be
required
to
contribute
to
the cost
of
theservices rendered(co-payment)

by
the probationofficer,basedupon

pay.

shall
submit
to an
evaluation for
mental healthcounseling
as
directed
by
theprobationofficer,
and
ifdeemed
necessary

mental healthprogramapproved

the

of

condition

supervision

computerwith internetaccess,thedefendant

computers
