JOB ANNOUNCEMENT Title: Intensive Supervision and Clinical Services Care Manager Salary: $32,000 - $36,000 per year

plus benefits Reports to: Project Manager (Intervention and Street Outreach Component) Status: Full Time, Non-Exempt Community Youth Center of San Francisco (CYC) provides education, leadership development, behavioral health, intervention, street outreach, and workforce development services to youth and their families in the community of San Francisco. CYC envisions empowering young people to reach their highest potential as individuals and to develop a positive self/cultural identity. Our mission is to empower and strengthen high-need Asian youth and their families by providing comprehensive youth development through education, employment training, advocacy, and other support service. Under the supervision of the Project Manager (Intervention and Street Outreach Component), the ISCS Care Manager is responsible for providing service to high risk Asian and Pacific Islander youth and their parents in schools, community centers and the Juvenile Justice Center through coordinated intensive individual and family care management and outpatient mental health counseling. The Care Manager is also responsible for the development and implementation of the life skills training workshops and support groups. DUTIES AND RESPONSIBILITIES: Conduct assessments to determent needs of the youth on probation and their families. Attend court hearings and provide advocacy in courts for youth who are adjudicated and their families. Develop individualized care plan and monitor implementation of treatment goals. Ensure service linkage to the mental health services including assessment and counseling sessions. Facilitate weekly youth support group and arrange community service opportunities. Advocate for clients to reduce recidivism contact with juvenile justice system. Provide individual & family care management and support and referral services. Facilitate meetings with related parties during the intervention/reintegration processes. Maintain official contacts to schools, community organizations, Department of Public Health, Juvenile Probation Department, Public Defenders and attorney offices. Develop cooperative relationships with school personnel, probation officers, parents and families, collaborative agencies and other service providers. Attend client review meetings, clinical meetings, and agency meetings as required. Work in conjunction with other CYC program to provide effective, comprehensive services. Facilitate meetings with related parties during the intervention/reintegration processes. Participate in trainings, team meetings and community meetings as necessary. Maintains accurate documentation and statistics on client services, and produces monthly reports for granting agencies. Support events, activities, and other projects that promote violence prevention and

reduction in the Asian and Pacific Islander community. Other duties as assigned by the Management. QUALIFICATIONS: Bachelors degree in social services or related areas plus minimum of two years of care management experience. Experience in conducting interviews and assessment with youth and parents. Experience working with San Franciscos diverse communities and neighborhoods. Experience in conducting support groups. Knowledge of San Francisco Juvenile Justice System. Knowledge of community based agencies serving youth in S.F. Strong interpersonal skills to work with youth, parents and probation officers. Excellent verbal and written communication skills. Strong organizational and collaboration skills. Bilingual/bicultural (Cantonese) a plus.
