SERVICE AGREEMENT
This contract has been entered on the 04.02.2009 by and between XXXXX Pvt. Ltdrepresented by Director / Chief Operating Officer, Mr. XXXXX hereinafter shall becalled as “
employer”.

“employee”.

The employee appointed as

in thisorganization w.e.f. 08.02.2009 agrees that she/he shall work for a period of Minimum
TWO
years starting from 08.02.2009.The company has appointed the employees in the category of Probation initially for a period of Six months, likely to be extended for a further period depending upon the performance in acquiring the knowledge in this XXXX Pvt. Ltd. hospitals.On this day of 09.02.2009, the employee hereby undertakes to serve the company in fullconscience and with his/her own free will. The employee has agreed as follows:-1.To serve the company for a period of 
TWO
years as per the agreed terms andconditions.2.To obey and abide by all the rules, regulations, service conditions and code of conduct of the company and of its Authorized officers and representatives issuedform time to time.3.The employee shall compensate the company the expenses incurred for impartingthe training and specialty skills, and also towards liquidates damages for non-compliance of this agreement.4.Termination of services by the company for whatever reason including for non-compliance of, as per agreed terms and conditions, and also on medical grounds.5.The company shall have the right to recover as its discretion the compensationdue to it from the employee.6.The company shall not be responsible for any interruption in or stoppage of learning as well as service of the employee due to exigencies of work or for causes beyond its control.7.The other conditions of appointment of the employee with the company asexpressly modified hereby shall remain unchanged and shall continue to be binding on the employee.2
