SUMMER CAMP POLICIES We, the undersigned parents/guardians of ______________________________ whose application is page 1 and 2 of this form, hereby apply to enroll this/these students in GLENDALE CENTRE THEATRE ACTING CAMP. In doing so, we acknowledge that we have read and agree to the following conditions or en rollment. 1. Drop off Time/ Attendance: It is understood that the doors to the theatre will be open 8:45 a.m. (8:00 a.m. extended care) and we agree to SIGN OUR CHILD IN EVERY DAY IN PERSON. We agree that not signing in our child is unacceptable. It is understood that class begins promptly at 9:00 a.m . It is understood that not only does tardiness impact my child's progress in the camp, but it af fects the other students as well and we agree that multiple absences and/or ex cessive tardiness will prohibit my child from being able to perform in the final show. (Simply due to lack of rehearsal time). 2. Pick-up Time: It is understood that camp ends every day at 2:00 p.m. ( 5:00 p.m. extended care). W e agree that we must sign our child out wi th the teacher in the lobby and that it is not acceptable to simply have my child sign themselves out and then walk to the car - we must sign them out in p erson. It is understood that after 2:15 p.m. (5:15 p.m. extended care), the staff will begin calling the parents and a $ 1.00 per min. child care fee will apply, payable in cash upon pick-up. 3. Extended Day: It is understood that the extended day will begin at 8:00 a.m., and it will end at 5:00 p.m. Extended day is childcare; no addi tional instruction will be provided. 4. Lunch: It is understood that the camp does not provide lunch for my child. Because the camp does not allow the students to leave the building during the day for lunch (even with a parent's note), it is understood that the students will need to bring a sack lunch every day . We agree that they must be physically signed out by their legal guardian in order to leave. We agree to write our child's name on their lunch bag. It is understood that the students will be required to help clean up after lunch and at the end of the day. 5. Snack: It is understood that, everyday, the theatre has a small nutritional break for the students. It is understood that parents are encouraged to pack a nutritional snack which the students may eat during this break in addition to their normal lunch. 6. Dress Code: It is understood that loose clothing that allows for movement is best and that the students should feel comfortable to move fre ely without reservation. We agree that bare mid-rifs, exposed underwear and mini skirts are not acceptable, nor are spandex shorts (unl ess under regular shorts). It is understood that ALL STUDENTS MUST BRING OR WEAR WHITE AND/OR BROWN RUBBER SOLED GYM/TENNIS SHOES WITH SOCKS EVER Y DAY and that sandals, flip-flops, boots, heels, flats, Crocs or BLACK-SOLED shoes are not allowed. 7. Medication: It is understood that students bringing any type of non-prescription medication must first submit a note to the director by a p arent indicating the frequency that the medication should be taken. We agree that once the student has submitted the note, the student may be allowed to carry the non-prescription medication on their person while at camp and is responsible for taking the proper dosages at the proper time. W e agree that all prescription medication must be provided in the original package and kept in the office, where the camp director will allow the student to take the medication. W e agree that students must be ultimately responsible for knowing when and how much medication to take and that the camp is not responsible for administering proper dosages at proper times to students (the same is applicable to asthma inhalers). 8. Conduct: It is understood that the camp and its employees do not discriminate based on gender , race, creed, ethnicity or religion. W e agree that the students must treat one another with the highest respect. W e agree that INAPPROPRIATE BEHAVIOR AND/OR PROFANITY WILL NOT BE TOLERATED AND THAT STUDENTS WILL BE DISMISSED FROM THE CAMP FOR THE REST OF THE SUMMER if behavior becomes an issue. It is understood that if a student violates camp p olicy, the parent will be contacted in a timely fashion and steps will be taken to correct the matter . We agree that if the child's behavior does not impro ve, the camp will dismiss the student and that in all cases, the camp has final say in any dismissal. It is understood that the following behaviors are criteria for immediate dismi ssal: disrespect toward faculty or staff member or anything that manifests contempt for camp authority; roughhousing, fighting, disruptive and/or threatening behavior , bringing any weapon of any kind to camp; threatening any student, teacher, staff member or parent; gang/crew involvement (dress, signs, language, etc.); being in possession or under th e influence of any illegal drugs or alcohol; smoking in the theatre or within 50 feet of the theatre grounds; theft of any theatre or personal property; damage of any kind to t heatre or personal property. We agree to speak to our children and remind them of the correct and expected way to behave while in school. It is understood that if at any time, a parent can no longer support camp policies, they are encouraged to withdraw their child. It is understood and we agree that parents and guardians should not upbraid, insult, o r abuse any member of the camp administration, faculty, parent, student or staff on or off the theatre premises. 9. Refunds: It is understood and we agree that no refund or reduction of tuition will be made for the withdrawal of this student after the first three days or should the student be dismissed from the camp due to misconduct or any other physical or behavioral problems. In the event of an accident or illness , the camp and parent, at the camp's sole discretion, may share in the unused tuition 50% each. 10.Disclosure: It is understood that the parents/guardians of students enrolling in camp, are obligated to inform the camp at the time of the application, of any physical or behavioral problems the child has, which have required professional treatment, and it is understood that the school may decline to accept such child, or may revoke such acceptance, when it is apparent the child may not have a successful experience in the camp or may cause other students to not have a successful experience in the camp. 11. Liability: We hereby release, and agree to indemnify and hold harmless the camp and all its of ficers, directors, employees, agents, and representatives whatsoever from any and all losses, claim, damages, liabilities, cost and expense (including, but not limited to attorney's fees) which they , or any of them, or student sustain or incur in any way arising out of or in conjunction with this student's participation in any camp activities. It is also understood that such con ditions also relate to acts of nature. 12. Lost/Stolen/Damaged Property: We agree that the camp is not responsible for the loss or damage to a student's personal belongings for any reason, and that th e camp strongly recommends that students not bring items of value to the camp.

SUMMER ACTING CAMP ENROLLMENT AND PAYMENT: Please fill out the application form and sign the Conditions of Enrollment (page 3 of this application). Select the payment pl an you wish. Enclose your check or credit card number (PLEASE NO CASH IN THE MAIL - YOU MAY PAY CASH ONLY IN PERSON). You may also fax your application to 818-244-5042. We accept check, credit card or cash (cash in person only). W e accept all major credit cards (Visa, Mastercard, Discover and American Express). PAYMENT MUST BE RECEIVED IN FULL. NO DEPOSITS. Refunds will be made up until 2 weeks before the start of their desired session. GLENDALE CENTRE THEATRE SUMMER ACTING CAMP CURRICULUM AND INFORMATION Thank you for your interest in the Glendale Centre Theatre's Summer Acting Camp for children and teens. In this program, your children are taught a variety of different skills in a safe and caring environment. The program runs five days a week from 9:00 a.m. to 2:00 p.m. for three weeks and is offered three times a summer. Students spend the day at the theatre and are involved in a wide range of classes specifically designed to stimulate their creativity and imagination. A summary of each of the classes is listed below . Note that some classes may vary slightly by session contingent upon teaching staf f. ACTING: Students will be exposed to various acting warm ups and exercises connecting movement and sound. They will also learn how to read a script, mark stage directions, memorize lines, audition and conquer stage fright! Actors will develop a strong u nderstanding of character development, acting techniques and monologues, as well as theatre etiquette. This class is all about being an actor . SINGING: At the beginning of each class, the students will warm up their voices and practice listening to each other as they work on learning how to perform a song, not just sing it. At the end of the session each group performs a Broadway-style musical number! DANCING: One of the most essential parts of being an actor is to know how to move and how to interact with others on stage. This class teaches basic movements with music and by the end of the session, each group performs a choreographed Broadway style danc e number on our stage! IMPROVISATION: In this class your child will be exposed to various exercises and games that help them act and react to what is happening in that exact moment. By working with others in the class they are able to create and expand their imagination in a fun and, at times, hysterical environment! IN THE WINGS: Masks, costumes, props, script writing and more! A fun journey into the behind the scenes aspects of theatre. SHAKESPEARE & STAGE COMBAT: Students will be drawn into Shakespeare's world through an interactive, exciting approach to the Bard's life, times and works. The students will have the opportunity to actually become Shakespeare's most memorable characters, as they are guided through his greatest plays. Also, basics of stage combat in a safe, fun environment. (Contingent on instructor availability) FINAL PRODUCTION: At the end of the session, the students participate in a show on our mainstage with full lights, sounds, and small set pieces. The final show is comprised of a series of scenes, dances, and songs where each student participates in one dance number, one singing number, and one scene. The show will NOT run longer than two hours (there is no intermission). Show dates are list ed below. WE STRESS THAT MISSING REHEARSALS WILL GREATLY IMPACT YOUR CHILD'S PERFORMANCE IN THE FINAL SHOW. We do not provide costumes for the final show - instead, all of the students dress in a camp t-shirt handed out to them the day of the show and their own jeans or jean shorts. All sets, lights, sound, and music are provided. CAMP HOURS: 9:00 a.m. - 2:00 p.m. EXTENDED DAY CHILDCARE: 8:00 a.m. - 5:00 p.m. AGES: 7 years - 15 years (no exceptions) Group #1 Age 7 (maximum 10 children) Group #2 Ages 8 and 9 (maximum 12 children) Group #3 Ages 10 and 11 (maximum 14 children) Group #4 Ages 11 and 12 (maximum 18 children) Group #5* 12 and older (maximum 18 children) *(These students will have the option to exit the camp with a headshot & resume) Ages may vary slightly per group dependent upon enrollment.
