ROLL CALL SALUTE TO THE FLAG RECORDS

The Records of February 26, 2013 were passed to Councillor Caraviello. HEARING 13-092 LOCATION OF POLES, ATTACHMENT OF FIXTURES AND UNDERGROUND CONDUITSGRANTS OF LOCATIONS MEDFORD, MASSACHUSETTS CITY CLERKS OFFICE You are hereby notified that by order of the Medford City Council, a Public Hearing will be given at the Howard F. Alden Memorial Auditorium, 85 George P. Hassett Drive, City Hall, Medford, Massachusetts at 7:00 P.M., on TUESDAY, March 5, 2013 on a petition by National Grid of Malden, MA. for permission to the locations of mains as hereinafter described for the transmission and distribution of gas in and after the following streets, lanes, highways, and places of City of Medford and of pipes, valves, governors, manholes and other structures, fixtures and 1

appurtenances designed or intended to protect or operate said mains and accomplish the objects of said company; and the digging up and opening the ground to lay or place same. Wherefore it prays that after due notice and hearing as provided by law, it be permission to excavate the public to be located substantially in accordance with the plan filed herewith marked GP-MED-837879-12-351 dated 12-24-2012 The following are the streets and highways referred to: Replace and relocate approximately 190 of 6 plastic main on Harvard Street and 160 of 6 plastic main on Winchester Street to accommodate new MWRA 30 drain to be installed The said National Grid shall comply with all applicable provisions of law and ordinances of the City of Medford to the enjoyment of said locations and rights Approved Chief Engineer Upon review of this petition, the Engineering Division finds that the work can proceed forthwith provided that no City-owned or private utilities or other structures are adversely For instance, if the crossing of the Citys water main over a pipe joint , the City will consider this an impact and may require National Grid to pay for replacing a section of pipe. An engineers plan depicting existing utilities must be provided to the City prior to National Grid receiving their street opening permit. Any sidewalk and other city property must be restored to existing or better conditions. That is sidewalk restoration is to be a replacement in-kind: if a concrete panel is disturbed then a new concrete panel must be installed. Before beginning work the contractor shall notify Dig Safe and shall obtain applicable permits from the Engineering Division. National Grid shall complete all restoration in accordance with City of Medford regulations and standards as well as for removing all debris related to its work Approved - Supt. of Wires Call 781-393-2425 for any accommodations/aids.
