5 x 12" cardstock(2) 5 1/2 x 4 7/8" cardstock5 x 5" designer paper CD LabelLarge bradEmbellishments and stamp set as desiredRibbonPaper Piercer or long reach hole punchScorer PicturesAdhesive
 
Score down on the long end of the 5 x 12" cardstock at 3/4, 4"and 9".Score 1" vertically down the 5 1/2" side of the 5 1/2 x 47/8" cardstock. (Placement of the cardstock on the scorer horizontally is the 5 1/2" side). See Diagram.Repeat with the second 5 1/2 x 4 7/8" cardstock.
