Lottery Registration Instructions
Please take a few moments to read the instructions on the front and back of this lottery form. Making sure that you have provided the proper information will allow the lottery committee to easily process your requests.

Our “program year” runs from September to June. Once you have participated in an activity, we ask that you not choose it againuntil the next program year. This way, we’ll be able to give as many families as possible a chance to enjoy each group.

If you have a particular need or restriction around registering for a group, feel free to speak with Rosemary or make a note on theform and we’ll see what we can do.

Please remember your first choice.
We will NOT call you if you get into your first choice. You will get a call to confirm your secondary choice or if you are put on a waitlist.

Feel free to list as many choices (in order of preference) as you wish. This will allow us to give you another choice if your first onis closed. If you only provide one choice and that group is closed you will be placed on a waitlist.

With all programs running through June 2009, any family new to the Network will have first priority in the new session. Thisgives new families the opportunity to join us. Their registration forms will be processed before the regular lottery.For office use only
