Guidelines
•
Please paste your survey below, giving as much detail as necessary to convey exactlywhat you want from each item.Spell out all responses as they should appear in the final survey. Do not useabbreviations. Do not leave anything opento interpretation by the production team.
•
Use ALL CAPS for notes to the production team. Use normal type for instructions that willbe displayed to respondents.
•
Indicate the response types, if not self-explanatory. Most common responses types are:Select only one; Select all that apply; Freeform text; Numeric value; Rank order; Table orslider (for scales). If using tables, please indicate any layout preferences. Include alldetails of range answers, if used.
•
Please place your item number in brackets. An additional permanent inTASC ID numberwill be assigned by the production designer and added to the item within the samebrackets. You will need to know this number in order to retrieve your collected data fromthe database.
•
Indicate in brackets your preferred numerical answer codes for each answer (usually, Yes/ No answers are codedas 1 and 0, respectively). These are the numbers you will retrieve from the database. If your survey is a test, indicate correct answers in bold or with an asterisk.
•
Indicate page breaks, section breaks, and numbering.
•
Specify particular items that require answers, can be skipped, or have jump patterns. If items have “jumps”? If so, please indicate “If answer=_____, jump to question _______.”
