Straight Talk is an audience-driven, culturally-appropriate newspaper produced monthly for Ugandan adolescents by Straight Talk Foundation, a leading Health & Development Communication NGO based in Kampala, Uganda.

Straight Talk appears in Uganda's lead national daily, _The New Vision_, and is posted to 25,000 schools, CBO/NGOs, health units, churches and mosques across Uganda, giving the paper an audience of nearly 1.5 million adolescents each month. Among other things, Straight Talk strives to help older adolescents manage sexual feelings, have healthy relationships, and make informed life decisions.

In this issue, readers are introduced to genital herpes, a serious STD with no cure that also makes one more prone to HIV infection. Dr Edith Nakku, a Straight Talk Foundation focal resource person, talks about transmission, prevention and management of genital herpes.

Genital herpes is exploding in Uganda.This map above shows herpes infection by region in 2004-5.Countrywide 44 out of every 100 people aged 15-49 have genital herpes. In Central Region over 50% of young people and adults are infected.
 I went to a hospital in Kabwoheand was told it was syphilis.They gave me treatment for syphilis but there was nodifference. I later went to Mbararahospital and got the sametreatment for syphilis but still had no change.They advised me to circumcisethat it would help me heal. I did,but the blisters kept coming againand again. Finally, at the healthcentre, I was told I had genitalherpes. Now blisters come every month.They are normally full of water and itchy. I tried local medicinebut it did not help.When you get this infection, youwill live with it for the rest of  your life. It is very uncomfort-able. Do not be deceived. Always have protected sex. I you find out that you have it, go for treatment and follow thedoctor’s instructions well."

Daniel, here is your answer.According to Dr Edith Nakku of the Mulago STD clinic, genitalherpes is a sexually transmitteddisease affecting many peopleincluding young people likeyou. It is caused by the herpessimplex virus (HSV).
The Sero survey showsthat on average 1 in every 5young people aged 15 to 19years has genital herpes.

What happens whenyou get infected?

STD Clinic, MulagoA person who gets infected withgenital herpes virus for the firsttime usually gets signs within 2 to10 days.
The virus will cause blisters or soresfilled with fluid in and around thevagina, penis, buttocks or thighs.These blisters may burst later to formwounds. The person may also haveslight itching or discharge in the genitalarea.
Many people who have genitalherpes do not know it. They maynot recognize or get any signs.
This means the virus that causes genitalherpes is in the body but is inactive(sleeping). This state of having the viruswith no signs can go on for many years.But when someone falls sick with malaria, flu or HIV ortheir immune system isweakened in anyway,the sores of genitalherpes can erupt.
Early signsusually last 2 to3 weeks
Early signs of activegenital herpes mayinclude:ItchingA burning feeling in thegenital or anal area.Pain in the legs, buttocksor genital area.Discharge of fluid from thevagina.Groups of fluid-filled swellingsappearing near where thevirus has entered the body(mouth, penis, or vagina, onthe cervix or in the urinarypassage or near the anus).

Genital herpes is spreadthrough sex and sometimesfrom mother to baby.Most people get genital herpeswhen they have unprotected

Small red bumps appear then developinto blisters, and then become painfulopen sores.After several days, the sores becomecrusty and then heal without leaving ascar.
Other signs
Fever, headache, muscle aches, painfulor difficult urination, vaginal discharge,and swollen glands in the genital area.
Recurrence
Even after the symptoms are gone, thevirus stays sleeping inside the nervecells. Later the virus will become activeagain, causing fresh blisters and sores.In most people, the virus can becomeactive several times a year. This comingand going of sores is called arecurrence.An infected person may know that arecurrence is about to happen by atingling feeling or itching in the genitalarea, pain in the buttocks or down theleg.Sometimes blisters appear that may bevery small and barely noticeable, orthey may break into open sores thatcrust over and then disappear.

Some people have only one or twooutbreaks in a lifetime. Others mayhave several outbreaks a year.Outbreaks are sometimes brought onby another illness, stress or having amenstrual period.In some cases, outbreaks may beconnected to sunlight.
Shedding without signs
Sometimes the virus can be active butnot cause sores that can be seen.Small amounts of the virus may be shedfrom around the place where infectionfirst took place. So the virus will beplentiful in fluids from the mouth, penisor vagina or from sores that you canbarely notice.Shedding is when particles of something drop off (for example, sometrees shed their leaves). You might notbe aware of the shedding but are highlylikely to infect a sex partner at this time.

Since then she had sex twice.She said she had never noticed anydisease on her boyfriend.They met secretly and in a hurry at afriend’s house. She is now worried.

,has been having recurrent woundson his penis and upper thigh for twoyears. He first noticed them afterhaving sex with his second girlfriend.Since then he has been getting soreson his penis and thighs once everytwo months. He has suffered a lotand has been getting treatment.The health worker put him onsyphilis treatment. But blisters stillcome back.

There is no cure. There is onlymedicine to reduce the pain andcause quicker healing of the sores.This medicine can also reduce howoften and how seriously they comeback.
Managing the sores
Keep the area of sores clean and dryto help healing and preventother infections fromdeveloping.Wear cotton underwear. Avoidnylon underwear.Wear loose fitting clothes.
If you have genital herpes...
Does a person suffering from herpeshave HIV?

Not always. Many people have genital herpesbut do not have HIV. But genital herpesincreases a person’s risk of HIV due to thesores.A person whohas both HIV and genitalherpesis more likely to pass on HIV to theirHIV-negative partner or partners.A person who has genital herpes and doesnot have HIVis more likely to get HIV fromtheir HIV-positive partner.In people with weak immune systems due toHIV, malaria or other health problems, thegenital herpes blisters may take longer to heal.Avoid touching the area during anoutbreak of sores. If you touch it,wash your hands before touchingother body parts like the eye.Do not have sex from the time youfirst feel any symptoms until thesores are completely healed. Makesure that the scab has fallen off andnew skin has formed where thesore was.Place lukewarm or cold cloths onthe area with the sores to reducepain.Take lukewarm baths. A girl can sitin a basin of water and urinatethere. This will stop the urine fromburning the area with sores.
Protect yourself from genital herpes by delayingsex. Commit yourself to having one partner, andlearn about protected sex, so that you are saferwhen you start sex. Consistent and correctcondom use reduces transmission of genitalherpes. Avoid sex completely or be extremelycareful about using a condom correctly if youhave sores. Be open and tell your partner thatyou have a problem.
The wounds may be bigger and morewidespread. In fact they may not go away atall.
What is the difference between HerpesZoster (kisipi) and genital herpes?

They are caused by different herpes viruses.Genital herpes is caused by the herpessimplex virus.Herpes Zoster is caused by the virus thatcauses chicken pox (
namusuna
).They are similar in that both Kisipi and genitalherpes are more common and more serious inpeople with HIV.
Many people confuse genitalherpes and syphilis and gettreatment for syphilis whenthey actually have genitalherpes.
Syphilis
is caused by abacteria,

 is caused by avirus
Genital herpes
 causes manysmall blisters that turn intopainful sores.
Syphilis
causes a single sorethat is painless.
Genital herpes
 causes manysores that are recurrent (goand come back).
Syphilis
 causes a single sorethat disappears on its own anddoes not come back (althoughthe syphilis bacteria is still inyour body and will create moreand very serious healthproblems if not treated).
Genital herpes
 has no cure. The virus staysin your body. There is only medicine whichcontrols it to an extent.
Syphilis
is completely curable withanitbiotics if treated early.

These are very different diseaseswhich need different care.Only 3% of young people andadults in Uganda have syphilis butalmost 50% have genital herpes. If you have a problem of sores onyour private parts, it is probablygenital herpes. These sores areopen doors for HIV.

eek help from a health worker. Heor she should be able to detectgenital herpes from a medicalhistory and a good examination.A blood test may have to be done.The genital herpes sores maynot be visible tothe naked eye.
