Problem Identification & Problem Prioritization Cues Subjective: Ang hirap huminga dahil sa ubo at sipon ko.

Barado kasi ang ilong ko tapos parang di nauubos ang plema ko kaya ubo pa din ako ng ubo, as the client verbalized. Objective: -Productive cough -Dyspnea -Colds -Facial grimace -Irritability -Changes in respiratory rate: 23bpm Subjective: Dahil sa kalagayan ko, nahihirapan talaga ako, huminga. Ubo at sipon ang nagpapahirap sa akin huminga, as the client verbalized. Objective: - Difficulty of breathing -Productive cough -Facial grimace Nursing Diagnosis Ineffective airway clearance related to retained secretions secondary to difficulty of breathing as manifested by productive cough Rank 1 Justification This problem is highly prioritized because it is an actual problem and gives the patient discomfort. The prioritized problem concerns with the breathing pattern of the patient which is included in the ABC which is an emergency life-threatening situation. This problem should be prioritized to achieve the needed amount of oxygen to be circulated in the body to perform tasks effectively. Oxygen is the main element that our body needs which we can get through breathing.

Ineffective Breathing Pattern related to accumulation of bacteria in the alveolus secondary to pneumonia

The problem identified is secondly prioritized since this affects the breathing pattern of the patient. This problem also needs to be resolve since this may cause threat to the heart which is the basic organ of the body that is responsible for pumping the blood throughout the body.

Subjective: Medyo hindi okay yung pahinga ko ditto dahil na din sa mga taong labas masok dito sa kwarto at dahil na din sa pagmomonitor ng BP ko.

Sleep deprivation related to uncomfortable sleep environment as manifested by decrease quality of sleep.

The problem identified is the last to be prioritized. Since sleep disorders increase the risk of serious and chronic health conditions, including depression, diabetes, heart disease and obesity. Untreated sleeping disorders can also negatively affect treatment outcomes for some diseases.
