INFORMED CONSENT FORM Collegiate Summer Baseball Player Development: The Effects of Mental Preparation, Sport Specific Skill Practice, Strength and Conditioning, and Video Analysis Utilizing the Baseball Assessment Testing System Scott Michael Colby, M.A., C.S.C.S. McKendree University Puget Sound Collegiate League You are invited to participate in a research study. The aim of this study, specific to collegiate summer baseball player development (PD), is threefold. The first aim is to investigate the effects of participation in a collegiate summer baseball league. Secondly, the effects of a four-pronged player development program will be investigated; pre-season goal setting (GS), pre-game practices (PGP), post-game self-evaluation (PGSE), and intermittent video analysis (VA) comprise the four-pronged player development program in this study. The final aim of this study is to investigate the effects of strength and conditioning (S&C) programming with an emphasis on concurrent training and power-endurance. A significant improvement between pre-test and post-test scores on the Baseball Athletic Testing System (BATS) would indicate a positive effect of collegiate summer baseball activities, sport-specific player development programming, and sport-specific strength and conditioning programming. INFORMATION Participants in this study include all of the 100-200, 18-25 year-old, males that come from various colleges, universities, and/or high schools to play collegiate summer baseball in the Puget Sound Collegiate League (PSCL) during the 2013 Season. The Baseball Athletic Testing System (BATS) will be used to compare pre-test and posttest participant scores on the following individual tests that comprise the test battery: demographics/anthropometrics, body composition, flexibility, muscular strength, power, agility, speed, anaerobic capacity, throwing velocity, and batted-ball velocity. The pre-test is scheduled for Sunday, June 2; and the post-test is scheduled for Saturday, July 27. BATS will be utilized as the Puget Sound Collegiate League (PSCL) Baseball Combine and each testing session will last approximately two hours. All PSCL Games, all sport-specific player development activities, and all sport-specific strength and conditioning activities will be completed between these pretesting and post-testing dates. The Baseball Athletic Testing System (BATS) pre-test and post-test data will be recorded and stored within a Microsoft Excel 2013 Spreadsheet. The difference between pre-test and post-test data will identify either a level of improved performance or a level of diminished performance. The sport-specific player development (PD) materials will be kept in a three-ring binder assigned by team. Binders will be distributed to each respective teams dugout prior to Puget Sound Collegiate League (PSCL) Games and collected immediately thereafter. Participation will be calculated according to completion of pre-season goals (GS) and completion of post-game Initials:_________________

self-evaluation entries (PGSE), both of which will be tied together within the mental preparation process. Pre-game practice (PGP) and video analysis (VA) activities will be tracked separately, utilizing a Microsoft Excel 2013 Spreadsheet to track the level of participation in these activities. Participation in strength and conditioning (S&C) activities will be tracked utilizing the same Microsoft Excel 2013 Spreadsheet and methods utilized to track player development (PD) activities. RISKS Any and all risks to participants of this study are inherent to those which are typically associated with normal, everyday, baseball activity. Throwing, catching, running, hitting for average, and hitting for power are considered to be the five tools associated with baseball performance. Baseball Athletic Testing System (BATS), sport-specific player development (PD), and strength and conditioning (S&C) activities designed to be utilized in this study present no additional risk than typical participation in Puget Sound Collegiate League (PSCL) activities do. Each participant will complete a Physical Activity Readiness Questionnaire (PAR-Q) Form, PSCL Waiver, and Research Consent Form prior to participating in any research activity. TREATMENT McKendree University and the Puget Sound Collegiate League do not "automatically" reimburse subjects for medical claims or other compensation. If physical injury is suffered in the course of research, or for more information, please notify the investigator in charge, Scott Michael Colby, M.A., C.S.C.S., at 701 College Road, Lebanon, IL 62254-1299, and/or 618-537-6933 and/or scolby@mckendree.edu. BENEFITS Benefits anticipated to be achieved from this research are as follows: A positive relationship between summer collegiate baseball participations, player development activities, strength and conditioning activities, and improved player performance, A better understanding of appropriate programming for collegiate summer baseball player development and strength and conditioning activities, and/or A quality collegiate summer baseball experience for participants that results in improved performance. CONFIDENTIALITY The electronic file containing all Baseball Athletic Testing System (BATS) data, sportspecific player development (PD), and sport-specific strength and conditioning (S&C) will be stored on an 8GB Gigaware Flash Drive. As is inherent to the sport of baseball, player game performance is recorded for analysis. I-Score Software is used to record player game performance in the Puget Sound Collegiate League (PSCL), and shared upon request with college baseball coaches and professional baseball scouts. BATS data may also be shared with college baseball coaches and/or professional baseball scouts upon request. Therefore, PSCL League Administrators, PSCL Coaches, and PSCL Players will have access to a centralized hard copy of the BATS results. This centralized hard copy of the BATS results will simply list raw data and will be kept in the game operations tent for the league; further distribution (i.e. copies, file shares via email, etc.) of the BATS results

COLLEGIATE SUMMER BASEBALL PLAYER DEVELOPMENT will not be allowed. The PSCL is a developmental collegiate summer baseball league that emphasizes teaching and learning, and this practice of sharing information falls in line with the league philosophy. For the purposes of statistical treatment, only the researcher will have access to the data in order to keep the results private and confidential. Steps will be taken to randomly assign an identification number to each participant in place of their name. This final data set will be statistically treated and results will be reported within groupings according to treatments. COMPENSATION For participating in this study you will receive no compensation. The Puget Sound Collegiate League (PSCL) Baseball Combine and its implementation of the Baseball Athletic Testing System are a complimentary benefit that comes with your registration fees to participate in the PSCL.
