Background

Pemeriksaan kekuatan otot merupakan tanda klinis untuk membantu diagnosa kelainan saraf maupun otot dan prediksi prognosa suatu penyakit

Procedure a. Introduction b. Self Hygiene c. Exposure

Memperkenalkan diri dan menerangkan maksud dan tujuan pemeriksaan Mencuci tangan Anggota gerak yang akan diperiksa terpapar secara baik dengan melepaskan sebagian pakaian yang ada atau seluruh pakaian Dimulai anggota gerak atas kanan dan kiri bandingkan Kemudian anggota gerak bawah kanan dan kiri bandingkan

d. Inspection

Observasi anggota gerak yang akan diperiksa pada posisi berdiri, jalan dan berbaring

e. Palpation

Apakah ada atropi, hipertropi Bagaimana tonus ototnya Apakah ada nyeri otot atau sendi Apakah ada keterbatasan ROM sendi

f. Movement

Posisi jalan Pasien dapat berjalan / kesulitan Bagaimana gaitnya

Posisi berbaring Apabila pasien mampu menggerakkan anggota gerak atas : 1. Pasien diintruksikan menggerakkan anggota gerak atas kanan dengan tahanan cukup kuat dari tangan pemeriksa bila mampu, maka nilai kekuatan otot = 5. Bila tidak mampu dilanjutkam pemeriksaan berikutnya Kemudian anggota gerak atas kiri dengan instruksi sama

2. Pasien diintruksikan menggerakkan anggota gerak atas kanan dengan tahanan ringan dari tangan pemeriksa bila mampu, maka nilai kekuatan otot = 4. Bila tidak mampu dilanjutkan pemeriksaan berikutnya Kemudian anggota gerak kiri dengan instruksi sama

3. Pasien diintruksikan menggerakkan anggota gerak atas kanan melawan gravitasi bila mampu, maka nilai kekuatan otot = 3. Bila tidak mampu melawan gravitasi, dilanjutkan pemeriksaan berikutnya. Kemudian anngota gerak kiri dengan instruksi sama

4. Pasien diintruksikan menggerakkan anggota gerak atas kanan dibidang

horizontal bila mampu, bila mampu nilai kekuatan otot = 2. Bila tidak mampu dilanjutkan pemeriksaan berikutnya Kemudian anggota gerak kiri dengan instruksi sama

5. Pasien diinstruksikan menggerakkan anggota gerak atas kanan, maka pemeriksa meraba belly otot adakah kontraksi otot, bila ada kontraksi nilai kekuatan otot = 1, Bila tidak ada kontraksi otot nilai kekuatan otot = 0 Kemudian anggota gerak kiri dengan instruksi sama

6. Dilanjutkan anggota gerak bawah kanan kemudian kiri dengan instruksi sama no. 1 s/d no. 5 a. d 1.

Learning Outcome

The examiner is expected to be able : 1. to document /evaluate the extremities muscle strength according to Lovett 2. to perform manual muscle testing thoroughly and correctly

Background

Manual muscle testing is a clinical sign to support the diagnose of neurologic and muscular disorders, also the prognosis of the diseases

Procedure a. Introduction b. Self Hygiene c. Exposure

Introducing and explaining the aim of the examination Wash both hands The extremities being examine are exposed Inspect and compare the right and left upper extremities Inspect and compare the right and left lower extremities

d. Inspection

Observe the extremities being examined on the position of lying,standing, and walking

e. Palpation

Is there any atrophy, hypertrophy How is the muscle tone Is there any pain of the musculature or joints Is there any limitation of the joint ROM

f. Movement

Walking position The patient is able to walk or any difficulties ? How is the gait ?

Lying position Is the patient able to move his/her upper extremities ? : 7. Instruct the patient to move his/her right upper extremity wih strong resistance of the examiner, if the patient is able to do it, so the MMT = 5 If the patient is not able to do it, continue with the next examination Examine the left upper extremity with the same instruction

8. Instruct the patient to move his/her right upper extremity wih mild resistance of the examiner, if the patient is able to do it, so the MMT = 4 If the patient is not able to do it, continue with the next examination Examine the left upper extremity with the same instruction

Instruct the patient to move his/her right upper extremity against gravity, if the patient is able to do it, so the MMT = 3. If the patient is not able to do it, continue with the next examination Examine the left upper extremity with the same instruction

9. Instruct the patient to move his/her right upper extremity on a horizontal plane if the patient is able to do it, so the MMT = 2 If the patient is not able to do it, continue with the next examination Examine the left upper extremity with the same instruction

Instruct the patient to move his/her right upper extremity, the examiner palpate the the muscle belly whether there is any contraction, if there is contraction, the MMT = 1, if there is no contraction, the MMT = 0. Examine the left upper extremity with the same instruction

10. Continue for the lower right & left extremities with the same instructions no 15. a. d 1.
