SITE/WORKSHOP HEALTH AND SAFETY INSPECTION/JOB HAZARD CHECK LIST AND APPRAISAL FORM The attached Check List

and Appraisal Forms contain 25 activities that are essential in maintaining a Health and safe Working Environment on a construction site or in a workshop. Each Activity contains a number of elements which shall be checked - off for conformity or, where necessary, defining and corrective action required. For the purpose of appraisal, points are allocated to each element, dependent on how effectively each elements is being implemented determines the number of points assessed to that element, based on the maximum listed. Should a particular element not be applicable then the points for that element are deleted and a Applicable Total obtained on which the appraisal will be based. These Check List shall be utilised as the basis for carrying out independent Safety Audits.
