Planning Ahead: Medical Directives & Emergency contacts
Life Care Planning is an important task for all of us, whether young or old, healthy or facing challenges. Through increasedawareness and access to information, Arizonans of all ages canmake their choices known about who will manage their affairs inthe event of an emergency. At Fellowship Square, Residents areencouraged to utilize a Pre-Hospital Medical Care Directive and post it on your refrigerator or on the inside of your front door.Sometimes this is called a DNR—Do Not Resuscitate. The Pre-Hospital Medical Care Directive is a document signed by you andyour doctor that informs emergency medical technicians (EMT’s)or hospital emergency personnel not to resuscitate you. Accordingto Arizona law, the DNR form is printed on letter-sized, orange paper. Blank DNR forms are available from the Activities Office inVilla 1, or by contacting Resident Services: 721-3006.In addition, Fellowship Square require all Residents to have acurrent Emergency Contact form on file with our Leasing Office.Please take a few moments to review that your Emergency Contactinformation is correct and up-to-date. The Emergency Contact formhas an area to update any medications that may have changedthroughout the year. This information is extremely helpful if emergency medical personnel are called and you are unable tocommunicate. Please make yourself a copy of your emergencyinformation and place it on your refrigerator; this information isvital in an emergency situation. Emergency Contact forms areavailable from the Leasing office in Villa 2, or by calling ResidentsServices at 721-3006.Additional life care planning forms and servicesare availablefree of charge through the AZ Attorney General’s website at:
