Community Pharm Monday February 4, 2013 Processing Scripts Types and origins: New scripts Refills Transfers (ie:

ie: from Wal-Mart to Walgreens) Faxed scripts e-scribe Points of review for new scripts: Patient info MD info Med info *if a pt asks to request a new script for them, the request must be handled with professional Judgment. Best thing is to have Pt call doctor and request them to call in script. Refills: May be requested for existing scripts if: Script is still in date Script is originally from that pharmacy There are refills authorized by MD Script is due to be refilled NOTE: When a pt does not have refills or the script is expired, a refill request may be sent to the MD Techs may request refills from MD Refill Renewal Request: Specific points should be covered during a refill request. Pt info MD info Med info (directions, quantity, strength, name) Last fill date Pharmacy contact info Once a MD has authorized a refill request from pharmacy, it may be treated as a new script. Also include on profile, who authorized the refill and date the refill was approved and filled. Techs may take refill authorizations via fax and telephone Notations to include with refill authorizations: Authorizing agent of MD Date authorized Number of refills authorized Transfers: A script must be transferred between chains in order to be filled and will be treated as a new script once transferred Scheduled meds have specific laws pertaining to being transferred. Sched II cant be transferred. III-IV can be transferred one time between chains.

Faxed Scripts: A md may fax in a script for a pt if needed and would be treated as a new script Consider the origin of the prescription (cover letter, or info written by doc, if not call md to verify) Scripts should be legible, if not all info should be verified Special considerations must be taken with scheduled drugs Note: if a script has be e-scribed/fax, pts do not have the authority to get a copy and take to another pharmacy E-Scribing MD may send script via the internet in the form of an e-script Special considerations must be taken for scheduled meds ( MUST have a signature)(no sched II) NOTE: if a script has been e-scribed, pt do not have the auth to get a copy and take it to another Pharmacy Its ok to take a script from MD assistant BUT must be tied to the MD. On script list MD first then prescribed by assistant. If PA writes for a contolled, they have to have their own DEA/DPS number.
