To provide affordable safety and health training opportunutues to
employers, employees and safety and health professionals.
CONFERENCE REGISTRATION FEE:

(After 5pm on 4/17 higher rate applies)

April 17 - May 15, 2009 - $270
(After 5pm on 5/15 onsite fee applies)
\u2022\ue000
Onsite fee $290 - cash or check only

After May 15, attendees must bring the registration form and
onsite fee with them to the conference. Registrations will not be
processed in-house after this date.

Registration includes conference materials, continental breakfast,
break refreshments, and lunch.
10-HOUR REGISTRATION FEE:
The fee for this course is $55 and is in addition to the
conference registration fee.
Registration includes 10-Hour course materials, break refresh-
ments and continental breakfast.
Payment can be made by check, VISA, MasterCard or IAT. Pur-
chase orders will not be accepted.CEUs have been applied for.
REFUNDS:
Cancellations must be submitted in writing by 5pm on May 1
2009in order to receive a refund. An administrative fee of $25
will be charged for all cancellations. Substitution of attendees
is allowed but noti\ue000cation must be received no later than Fri-
day, May 15, 2009. \u201cNo-shows\u201d will not receive a refund.
