 AMENDMENTS TO THE SOLICITATION1.The Section L clause entitled "TECHNICAL QUESTIONS (EP 52.215-110) (AP1984)" has been modified. The text is as follows:
Offerors must submit all technical questions concerning this solicitation inwriting to the contract specialist. EPA must receive the questions no laterthan
20 calendar days
after the date of this solicitation. EPA will answerquestions which may affect offers in an amendment to the solicitation. EPAwill not reference the source of the questions.
2.The Section L clause entitled "INSTRUCTIONS FOR THE PREPARATION OF A QUALITY MANAGEMENT PLAN (LOCAL LC-46-22) (FEB 2003) DEVIATION" has been modified. The text is as follows:
Each offeror, as a separate and identifiable part of its technicalproposal, shall submit a Quality Management Plan (QMP) setting forth theofferor’s capability for quality assurance. The plan shall address thefollowing:(a) A statement of policy concerning the organization’s commitment toimplement a Quality Control/Quality Assurance program to assure generation ofmeasurement data of adequate quality to meet the requirements of the Statementof Work.(b) An organizational chart showing the position of a QA function orperson within the organization. It is highly desirable that the QA functionor person be independent of the functional groups which generate measurementdata.(c) A delineation of the authority and responsibilities of the QAfunction or person and the related data quality responsibilities of otherfunctional groups of the organization.(d) The type and degree of experience in developing and applying QualityControl/Quality Assurance procedures to the proposed sampling and measurementmethods needed for performance of the Statement of Work.(e) The background and experience of the proposed personnel relevant toaccomplish the QA specifications in the Statement of Work.(f) The offeror’s general approach for accomplishing the QAspecifications in the Statement of Work.Additional information on EPA requirements for the Quality Management Plancan be accessed at the following: http://www.epa.gov/qualityl/qs-docs/r2-final.pdf

follows:
The plan described in Section L entitled Instructions for the Preparationof a Quality Management Plan (QMP) will be determined acceptable or notacceptable. An offeror that submits a plan that ultimately is unacceptable atthe time of award will not be eligible for a contract award.
*Note: The Sample QA Project Plan (QAPP) described in Section L is to be included asan addendum to the QMP and will likewise be evaluated on an acceptable/non-acceptablebasis.
4.The attachment entitled "INSTRUCTIONS FOR PREPARING PROPOSALS" has been modified. The text is as follows:
A. FORMAT AND CONTENT
Each offeror shall submit a written proposal in response to the solicitation. Written proposals shall consistof three separate volumes, including: (1) the offer; (2) the technical proposal; and (3) the price proposal. Theentire written proposal must be received at the place and time specified; including the signed and completedsolicitation (on Standard Form 33) and any amendments. Each offeror shall submit four (4) sets (i.e., 1clearly marked original, plus 3 copies of the technical proposal and cost/price proposal , in accordance withthe instructions set forth herein. The following table summarizes the format and content requirements for the written proposal.
