Parental Consent Form for Minors
[Name o PARENT/Guardian]____________________________________, reerred to asPARENT, is the parent and lawul guardian o,[Name o MINOR] ________________________________, a minor, and agrees:Los Banos Bualo Soldiers 9th and 10th Cavalry Association is organizing and presenting:Educational activities, living history and Re-enactment o Bualo Soldiers 9th and 10thCavalry.[Name o MINOR] _____________________________________has the permission o PAR-ENT to participate in this event and all activities thereo.1. Assumption o Risk. I, the undersigned, do hereby acknowledge that as a participantin the Los Banos Bualo Soldiers 9th and 10th Cavalry event, I am aware and reely andvoluntarily allow the minor named above to engage in a hazardous activity or recreationand enjoyment. I recognize that as a part o this activity other participants and the abovenamed minor will traverse dicult and dangerous terrain and obstacles, and that othersaround the above named minor will use various frearms and weapons, explosive materi-als and devices, and the above named minor will ride in hazardous transportation, andthat I reely and voluntarily assume any and all risk o injury associated with these activi-ties. For these reasons, I hereby waive and release rom liability and agree to hold harm-less the ollowing: Los Banos Bualo Soldiers 9th and 10th Cavalry Association, and/orsta, personnel, and volunteers o the above and all owners and operators o participatingvehicles, both modern and period, that may be used to transport or move personnel atthe event. Although acilities, rereshments, and other assistance may be made availableduring this event, I, as guardian am responsible or the above named minor’s health andsaety. I represent and warrant that the above named minor is in good health, physicallyft to engage in this activity, and have no medical condition which would oreseeably jeopardize the above named minor’s saety during such participation or be aggravated bysuch participation.

2. Release rom Liability. In consideration or the above named minor being allowed toparticipate in this event, I agree to exempt and release orever all personnel, ocers,members o the Los Banos Bualo Soldiers 9th and 10th Cavalry Association, event coor-dinators, event volunteers, and registered reenactors rom any liability, claims, demandsor actions, or causes o action whatsoever arising out o any damage, loss, or injury to meor the above named minor arising out o the above named minor’s participation in thisevent, whether such loss, damage, or injury results rom the negligence o the Los BanosBualo Soldiers 9th and 10th Cavalry Association their personnel, ocers, members oragents, or rom some other cause. I also agree to exempt and release orever the Los Ba-nos Bualo Soldiers 9th and 10th Cavalry Association its agents, ocers, and employeesand also the event coordinators, event volunteers, and registered reenactors, whether act-ing ocially or not, or any liability, claims, demands or actions, or causes o action what-soever arising out o damage, loss, or injury to me or my personal property or the abovenamed minor arising rom the above named minor’s participation in this event, whethersuch loss, damage or injury results rom the negligence o Los Banos Bualo Soldiers 9thand 10th Cavalry Association ocers, agents, employees.3. Indemnifcation. In consideration or the above named minor being allowed to partici-pate in this event, I agree to hold harmless and indemniy the Los Banos Bualo Soldiers9th and 10th Cavalry Association, its personnel, ocers, members, agents, employees,event coordinators, event volunteers, and registered reenactors, against any claim or suit(including any attorney’s ees, costs, and expenses) arising out o my participation in thisevent.I certiy that I am the parent and/or lawul guardian o the above named minor. I urtherstate that by signing this document I have ully read and understand its contents, and I amvoluntarily waiving rights that may be granted to me by ederal, state and local statutes,as well as the common law o the State o Caliornia and the United States o America.Finally, I state that I have read a copy o this liability release and have completed the waiv-er and have kept a copy or my own inormation regarding my minor/child,[Name o MINOR]_________________________________.
