Introduction
The questionnaire is to understand the existing setup in terms of Hardware, Software, Oracle Applications. The reader is assumed familiar with, although not an expert in database and Application and operating system concepts.

Server Architecture
This section is to get an idea about the existing hardware server Architecture. 0* How many server(s)? 1* Operation System? 2* Storage capacity, Redundant/ emergency storage 3* Network bandwidth/connectivity, protocol 4* Any Special Architecture? Like High availability etc 5* For production server provide the following: 6* Memory 7* Swap 8* Storage capacity 9* Typical client configuration 10*Server architecture document and Network connectivity document.

Database Environment
This section is to identify the existing database environment and Oracle Application environment. 11*What are the various versions of Oracle RDBMS are used? 12*How many database instances exist on each server? 13*Existing backup and recovery procedures/ tools. (RMAN, Omni backup, SQL backtrack and other popular backup utilities)? 14*Is the DB follows OFA? 15*What is the average # concurrent connections to each of the databases?
