Requirements in the Issuance and Endorsement of STCW’95 Certificate to Marine Engineer OfficersOFFICERS IN CHARGE OF ENGINEERING WATCH(Operational Level)I.
 
Certificate of Registration/STCW Certificate of CompetencyII.
 
Medical Certificate from DOH accredited medical clinic/hospital with QSS In place, using the DOH approvedseafarers pre-employment medical form, which must not be more than one (1) year from the date of Issuance.If submitted during application for STCW’95 Certificate, should not be less than fifteen (15) days prior to expirydate.III.
 
Transcript of Record with SO NO. ( For Category A only)IV.
 
One (1) year accumulated approved seagoing service during the preceding five (5) years.A.
 
Company Certification of Sea Services- shall include the type of vessel, rank occupied, vessels KW, vessel’s tradingarea and inclusive dates of sea service (letterhead with dry seal)B.
 
Seafarer’s Identification and Record Book (SIRB)- one photocopy each of pages one and two and other pagesreflecting sea service.(In case of lost/damaged SIRB, any acceptable substitute documentary proof of sea serviceand affidavit of loss shall be required.V.
