The following materials should be attached and submitted with your nomination: CITATION. In no more than 30 words, summarize the specific reason for making this award to your nominee. The award citation will be based on your summary. NARRATIVE AND VITA. Please provide a one-page narrative description of the significant Aspects of the nominees career, curriculum vitae, awards and honors, and other material you think will be helpful. REFERENCES. Please provide the names of at least four other individuals from whom you have already requested letters of reference for the nominee. All reference letters must be received by 15th September in order to be considered. These letters of reference should be directed to the appropriate award committee email. Diversity of support is preferred; not all letters should come from individuals associated with the nominees institution.
