Facilities: None Exterior Weather: Predicted: 12 degrees Celsius, chance of rain, low wind, overcast. Light: All natural. Sound:Traffic, occasional people walking by. Notes:Good flat surface for skating, smooth surface for dolly shot, no regular stream of traffic, not very busy road.

Have the managers/owners advised the site is free of any hazards? Yes I havechecked that it is free of hazards. Does the basic building structure appear sound? Yes. If the location is an operational facility are there emergency procedures available on site? It is not an operationalfacility. Have arrangements been made for the supervision of any children who will be on the site for your production? Therewill be no childrenon site duringmy production. Is the site free of hazards which may lead to slips, trips, falls? It will be beforewe go on site. Have any danger areas been clearly marked and/or taped? Theywill be. Are there appropriate means of emergency egress and communications such as lights, fire exits, operational telephone lines and signs? Wewill all havemobilesphones. Are the access and egress points suitable for carrying/transporting the film equipment/gear? Its a roadso thereare no accesspointsnecessary. Are guardrails or hand railings in place on raised platforms or potentially unstable areas (eg cliff edges, stair cases, etc)? Thereare no cliffs or edgesas it is a road. Is the surface stable enough for mounting the camera support? Yes. Have flats/props been checked to ensure they are stable and safe for crew and cast? Yes. Are fire extinguishers and/or other fire safety equipment required/available and in working condition? Theyare not required,as any fire or flammableitemswill not be used. Has the issue of smoking of cigarettes by cast and crew been addressed? (Where can they smoke? Is there provision for extinguishing and cleaning up butts? Theycan smokeon the otherside of the road, as it is all outside,andwe will bringbin bagsto collect butts. Are there hygienic and functional toilets and hand washing facilities nearby? Yes. Is there clean, drinkable water on site and enough running water for requirements? No, but we will takebottlesof water. Have you checked the location for lighting at the correct time of day to match the shoot? Yes, I visitedthe site at the sametimewe will be shooting. What time of day do you loose light? Around4:00pm. Is there a hospital within 20 minutes travel time? YesKingstonHospitalis very close.

Is there a First Aid Kit available and someone responsible for it? Yes, we can get hold of a first aid kit and I knowfirst aid. Does traffic control need to be arranged? No. Can pedestrians safely move around the area? Yesthey can still walkon the pavement. Will you have enough people/equipment to transport the equipment to the location? Yes. Do you have sufficient light for cast and crew to work in? Yes duringthe day it will be light enough. Has protection against predicted rain or strong winds been provided or planned for? Yes we will bringumbrellasand waterproofcoversto protectequipment. Have you provision for disposal/recycling of waste generated while on location? Yes we will bringplasticbin-bags. Will there be any detrimental impact on the environment by the cast, crew and the use of vehicles and equipment? No therewill not.
