Complete this form, providing the information requested in accordance with the requirements in the SRFP. Do not include attachments and/or URLs unless specifically requested in this SRFP. Completed proposal forms should not be more than 10 pages in overall length (not including the Executive Summary) using no less than a 10 pt font; the Province will not evaluate any pages that exceed this maximum page count.

Proponent Experience
The Proponent and/or its proposed subcontractor(s) should have the following experience: a)
 
Instruction - 5 or more years within the past 7 years (as of the closing date of this SRFP) instructional and facilitation experience with subject matter expertise in both adult education and the subject area (Discrimination Prevention); b)
 
Curriculum Development
 –
5 or more years within the past 7 years (as of the closing date of this SRFP) experience developing training curriculum (including Instructional Systems Design skills as well as subject matter expertise in Discrimination Prevention); and c)
 
Training Administration
 –
2 or more years within the past 5 years (as of the closing date of this SRFP) experience in administrative and logistics support including (but not limited to) arranging venues suitable for training, managing course materials, working within learning management systems, etc. Provide examples of projects that the Proponent and/or proposed subcontractor(s) have completed and include all the information requested below. Three tables have been provided; Proponents can copy and paste these tables to describe up to five projects that demonstrate how they meet or exceed the requirements listed in section 3 above.
