Introduction
Injury is the leading cause of death for persons ages 1 to 44, both in Utah and the U.S.1,2 In Utah,
violence and injury were responsible for an average of 1,650 deaths, 11,360 hospitalizations, and
154,550 emergency department (ED) visits per year from 2009-2011.3 These injuries are often
preventable. Accurate surveillance is crucial for the prevention of injury. For many years, injury data
have been available at the state and local health district levels. Data are now available for major injury
indicators at much smaller geographic levelsUtah Small Areas (refer to Appendix A for details about
Utah Small Area designation). Utah is made up of 62 small areas that were determined by population
size, political boundaries of cities and towns, and economic similarity. Utah Small Areas contain one or
multiple ZIP Codes. These areas were devised in 1997 by the Utah Department of Health to improve
data reporting at the community level. Utah Small Area data for injury was developed using the resources
of IBIS-PH (Indicator-Based Information System for Public Health) for Utah. This report presents injury
data by indicator at the small area level; for the purpose of this report, Utah Small Area refers to the small
area where an injured individual resides rather than the small area where the injury occurred.
Reporting injury data by Utah Small Area allows a more precise look at injury in Utah than was previously
possible. Each year, the cost of injury fatalities, hospitalizations, and ED visits increases. Decision
makers and stakeholders may find this report helpful in deciding how to allocate limited resources
to communities in greatest need. Reporting data by Utah Small Area provides a background for the
development and refining of evidence-based community health plans that target specific populations.
Using Utah Small Areas as the basic level of analysis will strengthen targeted prevention programs and
reduce costs by tying specific issues to a particular Utah Small Area rather than a much larger region.
This report can be used to promote community involvement and policy development for the planning and
implementation of injury-related interventions.
To ensure an accurate analysis with a sufficient amount of data, this report used several years of data
combined. This extensive report focuses on emerging issues such firearm-related deaths and the
increasing rates of poisoning (mostly due to misuse of prescription drugs) across Utah. A broad overview
of the major types of injuries in the U.S., Utah, and by Utah Small Area is also provided.

Appendix A:
How Utah Small Areas
Were Designated

Because ZIP Code boundaries are arbitrary geographic areas designed for the convenience of postal
carriers, they do not always correspond to other more meaningful boundaries, such as those of cities or
towns, school districts, neighborhoods, or political voting districts. ZIP Code areas may be heterogeneous
with respect to important health-related demographic variables such as income, education, and household
composition. Additionally, ZIP Code boundaries are not stable because they are subdivided as the
populations within them increase. Also, very importantly, ZIP Code and Utah Small Area boundaries may
not match health event (e.g., a disease or exposure) boundaries.
Although ZIP Code areas may be less than ideal, they are routinely included on many public health data
records or can be gathered easily on surveys such as the Behavioral Risk Factor Surveillance System
(BRFSS). Also, estimates of population size and other demographic characteristics are often available for
ZIP Code areas. When calculating disease and mortality rates, these population estimates are used as
the denominator, making them especially important.
Rates calculated from few events or in a small population typically show relatively large fluctuations with
small changes in the number of events. Such fluctuations create challenges for those trying to make
valid interpretations. For instance, it is difficult to assess whether rates, such as those in the indicators
included in this report, are either higher or lower than usual, or in relation to some standard such as the
state rate. There are a number of statistical approaches for detecting significant differences that are
discussed in the article by Haggard, et al.4 For this report, confidence intervals were used. Confidence
intervals describe the precision of an estimate and take into account the variability of the indicator, the
sample size, and the sampling method. Five to three years of data were combined to increase sample
size in the Utah Small Areas and improve the precision of the estimates.
One limitation in the use of confidence intervals to identify areas that are significantly different from
the state is the lack of independence of the Utah Small Areas and the state. The state includes the
Utah Small Area, so it is not technically appropriate for comparison. Another issue is that the state rate
also has a confidence interval. For this report, a Utah Small Area differed from the state rate when its
confidence interval did not include the state rate; however, the confidence interval for the state rate was
not considered.
In Summary
Utah Small Area analysis has many uses in public health. It can assist in targeting health promotion
efforts and examining health status in Utah Small Areas. It can also help to identify environmental, health
system, and demographic attributes that may be related to the health of a community.
Though the method described here is not perfect, it allows for the presentation of estimates by Utahs
Small Areas.

Appendix E: Methodology
The 17 indicators in this report come from a variety of data sources. The Traumatic Brain Injury (TBI)
indicator was created from an independent database maintained by the Utah Department of Health
Violence and Injury Prevention Program. The additional 16 indicators were created using data obtained
through queries of the IBIS-PH (Indicator-Based Information System for Public Health) web-based data
query system.
Because the BRFSS is a survey with its own methodological complexities, an entire section is devoted
to its description. The demographics section in this report contains demographic characteristics of the 62
Utah Small Areas. Demographic data come from U.S. Census data and BRFSS data.

Utah data sets (e.g., population estimates, deaths, health surveys, emergency events) and allows for
queries considering a variety of demographic variables. One of the benefits of using IBIS-PH to obtain
data is that the system allows for many data queries to be readily performed at the Utah Small Area level.
The purpose of IBIS-PH is to provide information to support evidence-based decision making, plan and
improve service delivery, evaluate health care systems, and inform policy decisions in Utah.
Data from the Utah Emergency Encounter Database (for indicators measuring Emergency Department
visits), the Utah Inpatient Hospital Discharge Database (for indicators measuring hospitalizations), and
the Utah Death Certificate Database (for indicators measuring fatalities) were all accessed for this report
from IBIS-PH. Indicators included in-state injuries and fatalities of Utah residents only. Deaths resulting
from war injuries are excluded from the data. The patients/decedents location of residence, rather than
the location where the injury took place, is the determinant of Utah Small Area.
Some cases may lack the ZIP Code information necessary to assign them to the appropriate Utah Small
Area, yet still have enough information to assign them to the appropriate health district. As a result, on
the individual indicator bar graphs, it is sometimes the case that the total number of injury/death cases in
a particular Utah Small Area may be fewer than the number of cases in the local health district.
ICD stands for International Classification of Diseases. It is a coding system maintained by the World
Health Organization and the U.S. National Center for Health Statistics (NCHS) which is used to classify
causes of death on death certificates and diagnoses, injury causes, and medical procedures for hospital
and emergency department (ED) visits. These codes are updated every decade or so to account for
advances in medical technology. Currently, the U.S. is changing over from the 9th revision (ICD-9) to the
10th revision (ICD-10). For all data years in this report, the standard is to use the ICD-10 coding system
to code fatal injuries and the ICD-9 coding system to code nonfatal injuries.
ICD-10 differs from ICD-9 in a number of respects: 1) ICD-10 is far more detailed than ICD-9, with nearly
8,000 categories compared to 4,000 categories; 2) ICD-10 uses 4-digit alphanumeric codes compared
to 4-digit numeric codes in ICD-9; 3) three additional chapters have been added and some chapters
rearranged; 4) cause-of-death titles have been changed and conditions have been regrouped; 5) some
coding rules have been changed; and 6) ICD-10 is published in three volumes compared to two volumes
in ICD-9.
Data used to create the injury indicators in this report are consistent with the standard injury case
definitions. Emergency department (ED) and hospitalization data were coded using ICD-9 injury codes
and case definitions are consistent with the Consensus Recommendations for Using Hospital Discharge
Data for Injury Surveillance (2003) developed by the State and Territorial Injury Program Directors
Association (STIPDA) Injury Surveillance Workgroup. The IBIS-PH query system is set up so that the
data can be readily queried based upon standard injury case definitions or individual ICD-9/ICD-10
code/s. ED and hospitalization data in the IBIS-PH system are based only on the first diagnosis code
recoded in the original data. Fatality data are coded using ICD-10 injury codes and case definitions are
consistent with the External Cause of Injury Mortality Matrix for ICD-10 found on the National Center for
Health Care Statistics (NCHS) website at http://www.cdc.gov/nchs/data/ice/icd10_transcode.pdf. Fatality
data in the IBIS-PH query system are based only on the underlying cause of death and do not take into
account immediate causes.
For each indicator following, all possible ICD codes that may be represented in the data in this report are
listed.

Indicator Maps
Maps were created using ArcMap 10.1 software by Esri. Base maps and boundary data were obtained
from the Utah Automated Geographic Reference Center (AGRC). Indicator data displayed on the maps
were accessed from IBIS-PH.
One injury indicator in this report, Seatbelt Use, comes from BRFSS data. The BRFSS is a telephone
survey that measures chronic disease, injury, and related health risk behaviors among adults ages
18 and older. It is coordinated by the U.S. Centers for Disease Control and Prevention (CDC) and
administered at the state level in all 50 states as well as in several U.S. territories. The BRFSS
questionnaire is modified each year by the CDC in collaboration with participating states and territories.

BRFSS Sampling Design
In the years 2002-2008, the Utah BRFSS telephone sample was disproportionately stratified by Utahs
12 health districts and by phone type (listed and unlisted landline telephone numbers). Rural health
districts with a comparatively lower population were sampled at a higher rate than urban health districts
with larger populations. This over-sampling of lower population districts allowed for the collection of
enough observations to produce reliable estimates within each district. Beginning in 2009, the BRFSS
data included both landline and cell phone respondents, which results in three phone strata. Further
enhancements included a new weighting methodology called iterative proportional fitting, or raking. This
methodology utilizes additional demographic information (such as education, race, and marital status)
in the weighting procedure. Both of these methodology changes were implemented to account for an
increased number of U.S. households without land line phones and an under-representation of certain
demographic groups that were not well-represented in the sample. More details about these changes can

The monthly number of telephone numbers sampled from each health district is routinely tallied to ensure
a target number of completed interviews each month in each district. Rural health districts were oversampled to obtain at least 500 interviews in three years per health district. In addition, urban counties
were oversampled in order to obtain sufficient numbers for Utah Small Area estimates. To facilitate
analysis of the Utah BRFSS data by Utah Small Area, a state-added question was included to collect
the respondents ZIP Code. Once a residence was successfully contacted, individual respondents were
randomly selected from all adults ages 18 and older living in the household. The selected adult, if willing,
was then interviewed in accordance with the BRFSS protocol.
Data Collection
Interviews were conducted monthly from the Utah Department of Health (UDOH) Survey Center by
professional interviewers employed by the UDOH. The Survey Center uses a computer-assisted
telephone interviewing (CATI) system to record respondent answers to the survey directly to a
computerized database. The system is programmed to help ensure accurate data entry. The interviews
were conducted during daytime and evening hours on weekdays and during daytime hours on Saturday
to ensure that selected respondents had ample opportunity to complete the survey. Fifteen attempts were
made to reach a phone number at different times of the day and on the weekend. Selected respondents
were given the opportunity to schedule a time to be called in order to complete the interview. Interviews
are routinely monitored to ensure adherence to strict BRFSS protocol. Monitoring is done electronically
so that both the interviewer and respondent can be heard, and the computer screen can be observed to
make sure responses are entered correctly without the interviewer being aware that he or she is being
monitored. The overall response rate during the period was about 66%.
Data Analysis
Sampling error: The BRFSS data were gathered from a sample of the Utah adult population.
Sampling error refers to the random variation that occurs because only a subset of the entire population
is sampled and used to estimate the finding for the entire population. It is often called margin of error in
popular use.
Non-sampling error: Sources of non-sampling error include idiosyncratic interpretation of survey
questions by respondents, variations in interviewer technique, household and item non-response to
questions, and coding errors. Respondents may have the tendency to underreport behaviors that are
undesirable, unhealthy, or illegal (e.g., drinking and driving). They may overreport desirable behaviors.
The accuracy of self-reported information is also affected by the ability of respondents to fully recall past
behaviors or health screening results. Unit non-response is when the person chosen for the sample
refuses to complete the survey. Non-response rates in the BRFSS have increased in recent years.
Conversely, overall response rates for the BRFSS have decreased over time. The Utah BRFSS Council

of American Survey Research Organizations(CASRO) response rate was 64.6% in 2010. In 2011, the
BRFSS switched to calculating American Association for Public Opinion (AAPOR) response rates rather
than CASRO rates and the sampling methodology included both landline and cellular telephones. In
2011, the AAPOR response rate was 52.2%.
Weighting: Design weights were constructed to account for differences in the probability of selection
(e.g., the number of adults, phones in a household). Iterative proportional fitting (raking) further adjusted
the design weights to account for additional demographic information (age by sex, race, education,
marital status, home ownership, sex by race, age by race, phone type, health district, age by health
district, sex by health district, and race by health district). This new methodology reduced bias and
improved estimates.
Rate estimates: Respondents who answered Dont know/Not sure or refused to answer were
excluded from the calculation of rate estimates. The SAS statistical package with SAS-Callable
SUDAAN computer software was used to compute rate estimates (both crude and age-adjusted) and
associated 95% confidence intervals using sample weights provided by the CDC. SUDAAN software
takes into account the complex BRFSS sample design in calculating unbiased standard errors for the
confidence interval calculations.

Traumatic Brain Injury (TBI)
The Traumatic Brain Injury (TBI) indicator in this report was created with data from the Utah TBI
database. The Utah Department of Health Violence and Injury Prevention Program collects TBI
information for the state of Utah through review of Hospital Discharge Data. To ensure a complete
TBI data set, cases were ascertained through the Utah Hospital Discharge Database. All acute care
hospitals within the state are required to report their hospitalization data annually to the Office of Health
Care Statistics.
TBI hospitalization cases include any injury documented in a medical record with one or more
of the following conditions attributed to head injury: observed or self-reported decreased level of
consciousness; amnesia; skull fracture; objective neurological or neuropsychological abnormality; or
diagnosed intracranial lesion.
These TBI cases exclude several conditions as follows: lacerations or contusions of the face, eye, or
scalp without other criteria; fractures of facial bones without other criteria; birth trauma; primary anoxic,
inflammatory, toxic, or metabolic encephalopathies which are not complications of head trauma; brain
infarction (ischemic stroke); intracranial hemorrhage (hemorrhagic stroke) without associated trauma;
airway obstruction (e.g., near-drowning, throat swelling, choking, strangulation, or crush injuries of the
chest); seizure disorders (grand mal, etc.); intracranial surgery; and neoplasms.
These conditions are coded using ICD-9 codes for non-fatal injuries. Medical records allow for up to nine
ICD codes per event. These conditions are coded using ICD-9 codes for non-fatal injuries and ICD-10
codes for fatal injuries. Medical records for both non-fatal and fatal events allow for up to nine ICD codes
per event. A case is counted as a TBI if at least one qualifying ICD-9 or ICD-10 code is present in the
medical record. The TBI ICD-9 and ICD-10 codes are as follows:
