Which policy is right for you?


If you're looking for a new collective healthcare insurance policy, you will nd a summary of CZ's policies in this handy package comparer. You can tell at a glance which package is best for you. If you're not looking for new collective insurance, you can check what is reimbursed by your current CZ healthcare insurance you can check this in the conditions and cover summaries. These are available from CZ Customer Service (0900 0949, local rate), at the Chamber of Commerce in Tilburg or online at www.cz.nl/voorwaardencollectief (information in Dutch).

You have to satisfy a number of conditions to be eligible for reimbursement. These conditions are not shown in this package comparer but in the general conditions and the summary of reimbursements for your insurance. The Insurance Terms and Conditions tell you, for example, if you need a referral from a healthcare provider, which indications are covered and if you need prior (statement of) approval from CZ.

Collective additional insurance


Do you want more comprehensive insurance than just a general policy? You can choose one of our collective additional policies: 'Basis Collectief', 'Plus Collectief' and 'Top Collectief'. Apart from these collective additional policies you can also choose from the packages that t in with your stage of life, such as 'Jongeren' and 50+. In the package comparer you can compare the different additional policies. The additional insurance 'Jongeren' already has cover for the dentist and/or orthodontist. Are you opting for a different additional policy but you want reimbursement for dental costs? On the last page you will nd an overview of the two dental policies you can choose from: 'Tandarts Collectief' and 'Uitgebreide Tandarts Collectief'. All the amounts in this package comparer are maximum amounts and apply per insured person per calendar year, unless stated otherwise. The maximum reimbursement is the (usual) market rate applicable in the Netherlands. It is a condition in every case that CZ has recognised the healthcare provider and/or institution. If there is a this healthcare is insured. You will nd a more detailed explanation on the back of the package comparer.

General insurance policies


At CZ you can choose from two general insurance policies: with the Zorg-op-maatpolis the healthcare provider/institute must have a contract with CZ. If this is not the case you may need to pay part of the cost yourself. If you choose a provider who doesn't have a contract with CZ the reimbursement will be as shown in your insurance conditions. In the case of the Zorgkeuzepolis the maximum reimbursement is the market rate applicable in the Netherlands. The rst column shows the reimbursements that apply to CZ's general insurance policies. The amounts are per insured person per calendar year, unless stated otherwise. Beside this is the 'Deductible' column. This shows whether the reimbursement falls under the deductible or not. Care and accommodation in hospital If you're unsure about the reimbursement for hospital treatment, for instance for non-contracted care, an independent treatment centre (ZBC) or reimbursement under the additional insurance, please call the Hospital Line first on +31 13 593 82 40. If your healthcare provider declares more than 112.50 per day you must pay the higher amount as an extra contribution. The personal contribution for psychiatric care without hospitalisation and for psychotherapy is per course of treatment and together a maximum of 200 per year.

Dentistry and orthodontics


The total reimbursement for dental care under the additional insurance is a maximum amount per year. If you have treatment, the reimbursement will be a percentage of the market rate applicable in the Netherlands up to the maximum amount per year. For example: you opt for the additional insurance 'Tandarts Collectief' and you have dental cleaning (other dental care), you will then receive reimbursement of 100% of the usual market rate in the Netherlands up to a maximum of 450.

These are mutually exclusive policies. CZ can determine which additional policies this concerns. or example: with the 'Jongeren' policy you're also insured for dental and orthodontic costs. This is why you can't also take out dental insurance with these policies.
