The doctor at the primary health centre is perhaps the only qualified modern medical personnel that most people in Chhattisgarhcan access. However the medical officer posted there has been trainedin a setting of relatively high availability of diagnostics and in a settingwhere a number of colleagues and seniors have been available forconsultation. Thrust suddenly into the milieu of the village primary health centre with little diagnostics and no one to consult it is very difficult for the medical officer to cope with clinical decision making.This book is meant to reach out to professionals in such a situationand guide them in their daily clinical work.The vast majority of diseases are self-limiting or can be treatedwith the knowledge and skills that a qualified medical doctor is trainedfor. Unfortunately because of lack of continuing medical educationprogrammes, because of lack of standard treatment guidelines,because of lack of diagnostics and above all because of lack of confidence the medical doctor often contents himself with symptomatictreatment and treatment of simple illnesses. All the rest are referred tohigher centres even when there is no need to do so. Of those who arereferred to higher centres for diagnosis, majority of them can afterwardsbe followed up and provided treatment at the primary health centrelevel, provided a referral system is in place. Unless the quality of curativecare provided is improved by the above measures, the public willperceive little difference between the curative care the health systemprovides and the curative care provided by the unqualified, hazardouspractice of the so- called village doctor. Another major function of a standard treatment guideline is -together with the essential drug list and the state drug formulary – torationalise medical practice. Today, there is an urgent need to protectthe population from irrational and hazardous therapy. Oftenunnecessary drugs and injections are given to patients merely to satisfy a wrong perception of health and health care. This is not only a majorcause of ill health and danger to life; it has also become a majorcontributor to poverty. Educating people about the hazards of irrationalcurative care and the profession itself not falling prey to such temptationsis another necessity of our times.
