Herpes zoster (immunocompetent):

Varicella zoster (chickenpox):
80 mg/kg/day PO qid x 5days (max 3200 mg/day)
Mucocutaneous HSVinfection:
750 mg/m2/day IVq8h or 15 mg/kg/day IV q8h x7 days.
Genital herpes or HSV(immunocompromised):
80mg/kg/day PO 3-5 times daily(max 1000 mg/day)
Herpes encephalitis:
birth-12 yrs: 60 mg/kg/day IVq8h>12 yrs: 30 mg/kg/day IV q8hInfuse each IV dose over 1hour. Adjust in renalimpairment. Patients should bewell hydrated to avoidurolithiasis.
Topical:
Apply to affectedarea q3h while awake for 7days. avoid

Neurocysticercosis:
Patientshould also receiveappropriate corticosteroid andanticonvulsanttherapy if indicated.<60 kg: 15 mg/kg/day PO bidfor 8-30 days, max 800mg/day
