Community Support Worker (full-time and part-time)

this opportunity is for you!
The Wabano Centre for Aboriginal Health is an award-winning Health Access Centre that provides holistic health services to First Nation, Inuit and Métis people living in Ottawa - one of the fastest growing and diverse urban Aboriginal communities in Canada. Wabano is a fast-paced, client-oriented organization that models the Aboriginal values of balance, respect and community interdependence. Wabano is looking for Community Support Workers to help seniors live in their homes with the best quality of life possible (part-time and full-time workers are needed).
Provides Client Care Regular community support will be provided
in the individual’s home for 2 hours per visit
on a schedule to be agreed upon by the client, CSW and Senior Team Lead. Working under the supervision of a Registered Nurse, duties will include:
Housekeeping (dusting, vacuuming, cleaning kitchens and bathrooms, changing beds, organization of personal items);
Shopping for groceries, house hold/personal items;
Preparing light meals as needed or requested by client;
Following the care plan, observing and reporting any substantial findings and or changes
to the client’s overall physical, emotional, mental, and environmental
 status to the Senior Team Lead;
Performing other tasks delegated by Registered Nurse;
Documentation into EMR system in order to monitors
client’s well being and report any
findings to Senior Team Lead and other members of the Senior Team as appropriate;
Providing emotional support and feedback to clients as well as their families and friends in an effort to optimally support the senior in a holistic way;
Continually commending and supporting seniors in their efforts to be independent in their home and encouraging positive changes as needed so they may remain in their homes as long as possible;
Providing respite relief to caregiver by offering companionship to client;
Working with clients to establish healthy living routines;
Acting as a support in recommending and facilitating participation in cultural activities and programs;
Actively supporting and contributing to an organizational and seniors team effort of caring for seniors in the most holistically way possible.
Work plan(s)
Ensure that all aspects of the work/care plan have been addressed and completed on time;
Maintain high quality service to clients in all aspects of the work/care plan. 2.
Participation in Wabano events is an integral part of community and organizational support. These include:
Annual General Meeting (AGM);
Wabano events (feasts, Community Health Day, Culture Night etc.) as directed by the manager.
The successful candidate will have the following qualifications:
College Diploma as a Community Support Worker or related program or an equivalent combination of education and experience;
Demonstrated ability to work effectively in a multi-disciplinary environment;
Strong communication and interpersonal skills;
Demonstrated ability to work with the Aboriginal population and sensitivity to Aboriginal issues;
The willingness to learn about Aboriginal culture to better understand and serve the clients of the Centre;
Experience or willingness to learn use of Electronic Medical Records;
Access to a reliable vehicle and drivers license;
Ability to work flex/alternating hours to accommodate program coverage;
First Aid or CPR training or willingness to complete;
Individuals with Aboriginal heritage will be given preference for this position (please self-identify).
