Accompanying this memorandum is the classified report of our investigation of errorsand
omissions inninetyapplications to the Foreign Intelligence Surveillance Court prepared bythe
FederalBureau of Investigation(FBI)and the Off ice of Intelligence Policyand Review (OIPR).
Thereportsetsf orthourf indings regardingthe cause or causes ofthose errorsand omissions.Italso
of fersseveralsuggestionsregardingways to minimize factual errors in Foreign Intelligence

occurredand how errorsmight be minimizedinthefuture. Accordingly, weauthorize you to
distribute thereport to thosesenior Department managers whoyou believehaveaneed to review
ourf indingsandsuggestions.
