Minimal Cytoplasmic Panel–
A Robust Test Design For Leukemia Immunophenotyping
 
The tube allows forSingle step staining of surface andcytoplasmic markers;Single step lysis and permeabilizationMinimal number of washes/steps anda quick time to result (<1hr);lesser variabity due to standardiza-tion of protocol across labs;dried down format to reduce reagentlossesNo additional antibody pipetting andtitration steps.It is hypothesized that by using thecytoplasmic minimal screening tube,alternate algorithms can evolve foracute leukemic blast lineage identifi-cation, thereby providing affordablepatient diagnostic options.The points below summarize somenovel features of this tube:1. Allows simultaneous staining ofcytoplasmic and surface antigens2. Takes less than one hour and re-quires no antibody titrations or pi-petting steps.3. Minimizes variability by reducingthe number of steps and allows forinter-laboratory standardization.Leukemias can be categorized as adisease that involves an unusual andmassive build up in the number of thewhite blood cells in the blood or thebone marrow. The decision for treat-ment is made by looking at the age ofthepatient,clinicalsymp-tomsandresultsfromspecificdiagnos-tic testson pa-tientblood orbone marrow samples. Two importantquestions that have to be answered tochoose the treatment and provideprognostic information for acute leuke-mic patients are:
What is the lineage of the leukemic blasts? And what spe- cific antigens do they express? 
Currently, answers to these questionsare obtained using information frommorphology, flow cytometry, cytochem-istry and cytogenetics of patient bloodor bone marrow samples (Figure). Flowcytometric immunophenotyping pro-vides key lineage and prognostic anti-gen expression information. Pres-ently, a panel of ~13-15 antibodiesis used for the flow cytometricimmunophenotyping of acute leuke-mias. Along with the other tests,the associated cost burden to thepatient is high.At ReaMetrix, we asked,
“1. How do we create standardized easy-to-use solutions for intracel- lular markers that are highly line- age specific? 2. How do we design affordable screening of acute leukemic blasts?” 
As a solution, we developed a ro-bust single tube assay for screen-ing blast lineage in less than 1 hourusing cytoplasmic markers. Thetube contains dried antibodiesagainst cytoplasmic MPO (Myeloidlineage), cytoplasmic CD22 or cyto-plasmic CD79a (B-Lymphoid), sur-face CD45 (Blast Gating) and cyto-plasmic CD3 (T-Lymphoid). Througha rational selection of antibodyclones, concentrations, fluoro-chromes, fixation and permeabili-zation agents, both surface andcytoplasmic markers can be stainedin a single tube with minimal proc-essing steps.
