1 PURPOSE & SCOPE

This standard establishes the safety requirements that need to be applied before
work is performed in confined spaces. It also establishes the processes that must be
followed for hazard identification and risk assessment of Confined Spaces prior to
entry commencement. The aim is to provide management controls so that people
are not exposed to risk when working in confined spaces.

This standard applies to all work in confined spaces at the Hail Creek Coal Project.
Notice: The identification of, and work carried out in Confined Spaces shall be in
accordance with Australian Standard 2865:2001

2 DEFINITIONS

2.1 CONFINED SPACE

An enclosed or partially enclosed space which: that is at atmospheric pressure during
occupancy and is not intended or designed primarily as a place of work, and

a) Is liable at any time to
(i) have an atmosphere which contains potentially harmful
levels of contaminant;
(ii) have an oxygen deficiency or excess; or
(iii) cause engulfment; and

b) could have restricted means of entry and exit.

Confined spaces may include, but are not limited to:

a) Storage tanks, tank cars, process vessels, boilers, pressure vessels, tank-like
compartments that only have a manhole for entry, ceilings and floor spaces;
b) Open-topped spaces such as pits or degreasers of more than 1.5 metres
depth; or
c) Pipes, sumps, sewers, shafts, ducts, drains, tunnels and similar structures.

2.2 CONFINED SPACE ENTRY

For the purpose of this standard: a person whose whole body, upper body or head is
within a confined space is considered to have entered the confined space. However,
this is not intended to prevent a person from inserting their hand or arm while holding
a test instrument or probe into the confined space as part of the evaluation prior to
entry.

2.3 CONFINED SPACE STAND-BY PERSON

A competent person assigned to remain on the outside of, and in close proximity to
the confined space and capable of observing (if practicable) and being in continuous
communication with those inside. In addition, where necessary, to initiate emergency
procedures and operate and monitor equipment used to ensure safety during entry
and work in the confined space.

2.4 CONTAMINANT

Any dust, fume, mist, vapour, gas or other substance in liquid or solid form, the
presence of which may be harmful to health and safety.

2.5 EXPOSURE STANDARD

An airborne concentration of a particular substance in the persons breathing zone,
exposure to which, according to current knowledge, should not cause adverse health
effects nor cause undue discomfort to nearly all persons. The exposure standard can
be of three forms: time weighted average (TWA), short term exposure limit (STEL) or
peak exposure limit.

The following terms are used in calculating levels of atmospheric contaminants:

a) TWA the average airborne concentration of a particular substance when
calculated over a normal eight hour workday, for a five day working week.
b) STEL a fifteen (15) minute TWA exposure which should not be exceeded at
any time during a work day even if the eight hour TWA average is within TWA
exposure standard. Exposure at the STEL should not be longer than 15
minutes and should not be repeated more than four times in a day. There
should be at least 60 minutes between successive exposures at the STEL.
c) Peak a maximum or peak airborne concentration of a particular substance
determined over the shortest analytically practicable period of time, which does
not exceed 15 minutes.

Note: For further information, refer to the National Occupational Health and Safety
Commissions Adopted National Exposure Standards for Atmospheric Contaminant
in the Occuptional Environment (NOHSC: 1003 (1991)

2.6 FLAMMABLE RANGE

The range of flammable gas or vapour (percentage by volume) in air in which an
explosion can occur upon ignition. Expressed by lower explosive limit (LEL) and
upper explosive limit (UEL).

Note: The concentration of the flammable contaminant in air either below the LEL or
above the UEL propagation of a flame does not occur on contact with an ignition
source.
2.7 LOWER EXPLOSIVE LIMIT (LEL)

In relation to a flammable contaminant, the concentration of the contaminant in air
below which the propagation of a flame does not occur on contact with an ignition
source.

2.8 RESTRICTED SPACE

A space that has been identified as requiring a Confined Space Entry Permit should
hot work or hazardous chemicals be introduced to the space.

2.9 SAFE OXYGEN LEVEL

A minimum oxygen content in air of 20.8 percent by volume under normal
atmospheric pressure, and a maximum oxygen content in air of 23.5 percent by
volume.

2.10 AUTHORISED ENTRANT

An employee who has completed formal classroom instruction in their roles and
responsibilities and the requirements of this procedure.

2.11 BLANKING OR BLINDING

The absolute closure of a pipe, line or duct by the fastening of a solid plate (such as
a spectacle blind or a slip plate) that completely covers the bore and that is capable
of withstanding the maximum pressure of the pipe, line or duct with no leakage
beyond the plate.

2.12 DOUBLE BLOCK AND BLEED

The closure of a line, duct or pipe by closing and locking or tagging two in-line valves
and by opening and locking or tagging a drain or vent valve in the line between the
two closed valves.

2.13 EMERGENCY

Any occurrence (including failure of hazard control or monitoring equipment) or event
internal or external to the permit space that could endanger the confined space
entrants.

2.14 ISOLATION

The process by which a confined space is removed from service and completely
protected against the release or energy and material into the space by such means
as blanking or blinding, misaligning or removing sections of lines, pipes, or ducts, a
double block and bleed system, lockout and tag out of all sources of energy, or
blocking or disconnecting all mechanical linkages.

2.15 LINE BREAKING

The intentional opening of a pipe, line, or duct that is or has been carrying flammable,
corrosive, or toxic material, an inert gas, or any fluid at a volume, pressure, or
temperature capable of causing injury.

2.16 TESTING

The process by which the hazards that may confront entrants of a confined space are
identified and evaluated. Testing includes specifying the tests that are to be
performed in the confined space.

2.17 ENTRY CONTROL BOARD

Designated Board for attaching personal padlocks prior to entry into confined space.

3 ROLES & RESPONSIBILITIES

3.1 SITE SENIOR EXECUTIVE (SSE)

The SSE will:

Ensure effective implementation of this standard.

3.2 PROJECT CONSTRUCTION MANAGER

The Project Construction Manager will ensure that:

Site Safety Standards and Procedures are developed and maintained for the
management of hazards associated with Entry into Confined Spaces;
Sufficient resources are provided to ensure compliance with the requirements
of this Standard.

3.3 PROJECT SAFETY MANAGER

The Project Safety Manager will ensure that:

Appropriate training programs are provided for personnel required to enter into
and/or work in Confined Spaces;
Appropriate training and assessment is provided for those required to Issue
Confined Space Entry Permits;
Atmospheric sampling and testing equipment is serviced and calibrated at least
once per year, or more often if required, and records are maintained of such
servicing and calibration;
Atmospheric sampling and testing equipment is made available for personnel
required to enter into confined spaces;
Only gas detectors that have been properly calibrated and fitted with new
batteries are issued;
Rescue squad personnel are trained to rescue from confined spaces;
A site register is maintained of those authorised to Issue Permits;
This document is maintained within the site Safety Management System.

3.4 LINE MANAGERS

Each Line Manager will ensure that:

Confined or Restricted Spaces are identified within their area of responsibility;
A register of Confined and Restricted Spaces is maintained;
Identify and appoint sufficient, trained and competent persons and entry
Supervisors to Issue Confined Space Entry Permits and define their area of
authority to issue, and
Persons required to Issue Confined Space Entry Permits are retrained, tested,
and have Authority to Issue Permits renewed as required.

3.5 ENTRY SUPERVISOR

Knows the hazards that may be faced during entry and consequences of the
exposure;
Ensure that final safety checks have been performed prior to entry and verify
that all permit conditions are applied.
Verify that the atmospheric testing requirements documented on the permit
have been conducted and that all procedures and equipment specified by the
permit are in place before accepting the permit and allowing entry to begin;
Verify that rescue equipment and resources are available and that the means
for summoning them are operable;
Determine, whenever responsibility for a confined space entry operation is
transferred and at intervals dictated by the hazards and operations performed
within the space, that entry operations remain consistent with terms of the entry
permit and that acceptable entry conditions are maintained;

Ensure that Entry Control Boards are available and used.
Ensure persons entering Confined Space are authorized to do so.

At the completion of shift ensure that:
- The site is left in a clean and safe condition;
- All personnel have departed the space;
- All entry points are barricaded and signposted;
- All personnel have removed their Personal Locks and Danger Tags.
At the completion of work ensure that a pre closure inspection is undertaken (if
required).

3.6 PERMIT ISSUERS

Persons Authorised to Issue Confined Space Entry Permits will ensure that:

They review the permit and identify the means, procedures and practicees
necessary for safe confined space operations;
They understand the nature of the hazardous work for which they issue
permits;
They only Issue Permits for which they are authorised to Issue;
Persons working under Permits are aware of the hazards involved and the
precautions they must take;
A stand-by person is stationed at the entrance to the confined space with the
appropriate fire / rescue equipment at hand;
All necessary controls have been established before the work starts by
inspecting the work site and reviewing the relevant risk assessment and
subsequent Work Instructions;
Confined spaces are properly decontaminated, cleaned and purged (as
required) and the atmosphere is not explosive or toxic and has an oxygen
content of between 20.8% and 23.5%.
Atmospheric testing / sampling equipment is tested for response prior to entry
to the confined space by comparing readings of air outside with that inside the
confined space;
All sources of energy with potential for injury are isolated and locked out prior to
work commencing;
All controls and precautions in relation to the risk assessment are stated on the
Permit;
The Confined Space Entry Permit has been signed off before any associated
equipment is returned to service;
All people and equipment have been withdrawn from the work site and
equipment is safe to return to work before closing the Permit;

They remain competent in the use of monitoring atmospheres using site
sampling equipment; and
Conduct audits and inspections to ensure compliance with requirements of this
procedure.

3.7 AUTHORISED ENTRANTS

Authorised entrants are required to perform Confined Space Entry will ensure that:

Confined Space Entry Permits are obtained and applied where required;
Risk assessment is conducted prior to commencement of Confined Space
Entry;
They understand the requirements of the Confined Space Entry Permit and do
not deviate from the Instructions and Procedures outlined on the permit;
They satisfy themselves that all necessary precautions are taken and hazards
are effectively controlled before work commences and whilst performing such
work;
All energy sources are identified and effectively isolated before commencing
work;
They sign-on to the Permit, as required;
Confined spaces have been decontaminated, cleaned and purged, where
required, prior to entry;
The atmosphere inside the confined space is not explosive or toxic and has an
oxygen content of between 20.8 and 23.5%;
The quality and temperature of the air inside the confined space is continually
monitored and is re-assessed prior to re-entry where the space has been
temporarily unoccupied for in excess of thirty minutes;
No item / equipment is taken into confined spaces that might increase the risk
to those working within, eg: cigarette lighters, chemicals, etc;
All electrical equipment, welding cables and oxy / acetylene (fuel) hoses and
handpieces are removed from confined spaces whenever there is a significant
break from work, eg: crib, smoko, etc;
They leave the space immediately if the atmosphere or safe working conditions
deteriorate, and notify the person who issued the permit of any changes or
hazards existing;
They signify their occupancy of the space by attaching their personal lock and
danger tag on the Entry Control Board provided, and
They notify the permit issuer at the completion of work so that the permit can
be closed off.

3.8 ENTRY CONTROLLER

Confined Space Entry controller will ensure that:

They understand the nature of work to be carried out, the likely hazards and the
necessary precautions;

They read and understand the details of the Permit and other associated
Procedures and Instructions related to the task;
The Permit documents are retained at the work site for the duration of the work
and compliance to the Permit is monitored;
All persons involved in the task are notified of any variations, ie: Permit Issuer,
Supervisor and personnel engaged in the task;
A reliable means of communication is maintained between themselves and
those inside the confined space. Visual (where practical) and audible (use radio
if necessary) communications are preferred;
They stay in attendance at all times unless relieved by an authorised person;
They monitor those entering and leaving the confined space;
Appropriate rescue / emergency equipment is at hand and in ready-for-use
condition;
They stop the work any time they think a dangerous situation may be present (if
unsure, stop the work and seek advice);
They do not attempt rescue of persons inside the confined space themselves
but initiate site emergency procedures, should this be required;
Equipment outside the confined space that provides services to those inside is
monitored, eg; power sources, welding and gas equipment, ventilation systems,
etc.

4 PROCEDURES

4.1 REQUIREMENTS

4.1.1 Identification

Confined spaces must be identified and signs erected at the entry points denoting
that a permit is required prior to entry. Where signage is impractical, other means of
highlighting the dangers need to be used.

4.1.2 Permit System

Entry to a confined space must only be allowed after a written approval, in the form of
a permit, has been issued by a competent person, whom is authorised to issue such
permits.

The permitting process must include the following elements:

A risk assessment, including the need for a competent person to assess such
things as oxygen levels, contaminants, temperature, and concentrations of
flammable substances;
Isolation procedures for contaminants and other energy sources;

The requirements for breathing apparatus;
The sign-in and sign-out of all persons entering the confined space;
Display of the permit and the Confined Space Entry Board;
Communication equipment;
Barricading and appropriate signage and danger tags;
Rescue plan and equipment;
Standby person.

4.1.3 Other Requirements

All persons required to work in a confined space, or to act as a standby person, must
be trained, competent and tested.

Specific safe work procedures must be developed for work activities that are more
hazardous when carried out in a confined space than elsewhere. These activities
would include hot work (cutting and welding), chemical cleaning, steam cleaning and
abrasive blasting.

The standby person will have no other duties and is to be positioned outside the
confined space entry point at all times while personnel are within the space.

Where the risk assessment has identified the need for ventilation, then this must be
covered by a documented procedure.

4.2 COMPETENT & AUTHORISED PERSONNEL

Employees required to undertake entry or work within a confined space shall be
currently competent in the tasks that they are required to perform and in the site
compliance standards and procedures relating to Confined Space entry.

To be currently competent in Confined Space Entry, a person shall have
successfully completed appropriate site approved training in confined space entry
within the past 12 months.

Only persons who are currently competent and duly authorised to Issue Confined
Space Entry Permits shall authorise (issue) such permits.

4.3 CONFINED SPACE ENTRY PERMIT, PROCESS & APPLICATION
(ANNEXURE 1)

Where a confined space entry is required the Entry Supervisor shall complete Part A
of the Confined Space Entry Permit and submit the permit to the Permit Issuer.

The Permit Issuer shall review the Permit and identify the means, procedures, and
practices necessary for safe permit space entry operations in Part B of the Permit,
including, but not limited to, the following:

Specifying acceptable entry conditions;
Define the isolation requirements and instigate an Isolation List in consultation
with the appropriate Isolation Authority (Mechanical or Electrical);
Define any purging, inerting, flushing, or ventilation requirements as necessary
to eliminate or control atmospheric hazards;
Detail the pedestrian, vehicle, or other barriers as necessary to protect entrants
from external hazards;
Define the atmospheric assessment requirements necessary to ensure that
atmospheric conditions in the permit space are acceptable for entry throughout
the duration of an authorised entry;
If necessary the rescue and emergency services that can be mobilised and the
means (such as the equipment to use and the numbers to call) for mobilising
those services;
The communication procedures to be used by entrants and attendants to
maintain contact during the entry;
Equipment, such as personal protective equipment, atmospheric monitoring
equipment, alarm systems, and rescue equipment, to be provided;
Any other information whose inclusion is necessary, given the circumstances of
the particular confined space, in order to ensure employee safety.

4.4 CONFINED SPACE ENTRY PERMIT APPROVAL & VALIDITY

1. The Permit Issuer once satisfied that:

All the requirements of Part B of the Permit have been identified and put
in place
The isolations stated on the Isolation List have been put in place;
And any associated Permits are authorized, shall then complete the
authorisation to commence work section at Part C.

2. The Entry Supervisor for the confined space entry shall acknowledge the
Confined Space Entry Permit conditions and the requirement for all employees
undertaking the work to be informed by signing the Acceptance section at
Part C.

3. Prior to starting work the Entry Supervisor shall ensure that:

Where a lockout and tag-out is in place all personnel have placed their
personnel Locks and Danger Tags and have signed onto Permit or Sign
On/Off Continuation Sheet for extended Permits, and
An atmospheric assessment has been undertaken.

4. A Confined Space Entry Permit is valid for a maximum of 14 days and must
either be handed over to a new Permit Holder or handed back at the end of
shift.

5. The Entry Supervisor or Permit Issuer shall terminate entry and cancel the
permit when entry operations covered by the entry permit have been
completed, or when a condition that is not allowed under the entry permit arises
in or near the confined space.

4.5 CONFINED SPACE ENTRY PERMIT REVALIDATION &
RELINQUISHMENT

1. Re-validation at shift commencement and relinquishment at shifts end is
required for all Confined Space Entry Permits.

2. The Entry Supervisor shall ensure that the Original copy of the Confined Space
Entry Permit is relinquished to the Permit Issuer upon the completion of works
for the shift.

3. The Entry Supervisor shall relinquish the Confined Space Entry Permit by
signing the relinquishment section of the original and duplicates of Confined
Space Entry Permit and any associated Permits (held by the appropriate
Authority) conditional upon:

All personnel have left the confined space and all openings to the confined
space are bunted and signposted with Danger Signs to prevent unauthorised
entry;
Where a lockout and tag-out is in place all personnel have removed their
personnel Locks and Danger Tags;
All personnel have signed off the Permit or Sign On/Off Continuation Sheet for
extended Permits;
The site has been left in a clean and safe condition.

4. At the commencement of shift the Entry Supervisor shall re-validate the
Confined Space Entry Permit, and associated Permits, by signing the re-
validation section, of the original and duplicate copies of the Confined Space
Entry and any associated Permits.

5. All re-validations shall be initialed by the Permit Issuer after an initial
Atmospheric Test has been undertaken and the results logged onto an
Atmospheric Testing Sheet.

4.6 CONFINED SPACE ENTRY PERMIT CLOSURE & HANDBACK

1. At the completion of all work associated with the Confined Space Entry Permit
the Permit Holder shall, prior to closing the confined space, request permit
authorisation to do so from the Permit Issuer.

2. Prior to closure of a confined space the Permit Issuer shall inspect the confined
space to ensure it is ready for closure.

3. The Permit Issuer shall then sign the closure section on the Original copy of the
Permit.

4. The Entry Supervisor shall then sign handback section of the Original of the
Permit.

5. After completion of the closure, and after confirming all isolations associated
with the Permit have been removed, the Permit Issuer shall cancel the original
copy of the Confined Space Entry Permit.

6. Prior to the cancellation of the Permit the Permit Issuer shall sight the original
Isolation List to confirm all isolations have been removed and file the original of
the Confined Space Entry Permit and Isolation List.

4.7 CONFINED SPACE ENTRY GENERAL REQUIREMENTS

4.7.1 Access

Access openings into the confined space must permit rescue of all personnel who
enter the confined space. These access ways into and out of the equipment and
surrounding area must be kept clear of any obstructions.

The application of rescue equipment including stretchers shall be identified in the
Confined Space JHA.

It may be necessary to provide scaffold platforms and stairs or ladders to the access
levels to gain safe convenient access for work and for rescue purposes.

4.7.2 Atmospheric Testing

The Permit Issuer shall define on the Confined Space Entry Permit any atmospheric
testing requirements. As a minimum, an Authorised Gas Tester must perform an
atmospheric test prior to entry at the commencement of each shift and record the
result in Part B of the Confined Space Entry Permit.

The Permit Issuer shall identify the frequency of ongoing atmospheric tests if the
nature of the confined space or the work being performed indicates this is warranted.
Atmosphere testing and monitoring shall be carried out consistent with hazards

Ongoing atmospheric tests results shall be recorded by the Authorised Gas Tester in
Part D of the Confined Space Entry Permit.

As an absolute minimum, oxygen content and flammables content (% LEL) are
tested at each atmospheric assessment. Other contaminants potentially present shall

be tested prior to entry and when any repeat atmospheric tests are identified as
being required by Hazard Assessment or change of conditions.
Prior to entry, the atmosphere of a confined space must meet the following minimum
requirements:

The atmosphere must contain a 0% concentration of Lower Explosive Level
(LEL);
The atmosphere contains an Oxygen (O2) concentration of no less than 20.8%
and no greater than 23.5% by volume;
The concentration of any other atmospheric contaminant is no greater than the
Exposure Standard expressed as a Time Weighted Average (TWA); and
There is no potential for the level of atmospheric contaminant to fall outside the
above ranges.

4.7.3 Electrical Equipment Inside a Confined Space

If electrical equipment must be used the following precautions shall be taken:

32v lighting is to be used
Test and locate Residual Current Devices (RCDs) outside the space, if not
fitted elsewhere in the circuit;
Ensure all leads that must enter vessels and tanks are routed through a nozzle
or opening other than the access or hatch, wherever possible;
Ensure all electrical equipment is shutdown when the confined space is not
attended (Crew breaks etc);
Ensure all electrical equipment and leads are inspected and checked for
cracks, cuts and breaks in insulation. Have it checked by an authorised person
and faults rectified if any doubt of the condition exists;
All electrical equipment shall display current inspection identification;
Check all welding leads for cracks, cuts and breaks in insulation. Have it
checked by an authorised person and faults rectified if any doubt of the
condition exists;
Ensure shutdown switches for all equipment is readily available to the outside
Entry Attendant.

4.7.4 Entry Controller

An Entry Controller who has been trained in resuscitation shall keep the worker(s)
and work area under constant observation. Effective communication procedures
must be established between the worker(s) and the observer.

The Entry Coller shall be made aware of the Procedure and / or Work Instructions to
ensure compliance is maintained.

An Entry Controller must be positioned at the single designated entry point to the
confined space prior to any entry and during entry to the space.

Under no circumstances shall the Entry Controller enter the confined space or
attempt a rescue.

4.7.5 Job Hazard Analysis (JHA)

A JHA addressing the task and the specific hazards must be undertaken prior to any
confined space entry. The JHA must be made available to each employee entering
the space and discussed as part of task meeting.

4.7.6 Hot Work Inside a Confined Space

Caution must be exercised at any time hot work is required in a confined space as
the work process itself may render the atmosphere hazardous even though initial
tests were satisfactory.

In addition to the normal precautions associated with Confined Space Entry and hot
work the following conditions also apply:

A Hot Work Permit is required for all Hot Work activity carried out in a Confined
Space;
Continuous atmospheric monitoring maybe employed for high risk hot work
such as welding in poorly ventilated areas;
Ensure adequate ventilation to reduce welding fumes and where necessary
provide additional Personal Protective Equipment;
Suitable respiratory equipment shall be worn to mitigate the risks associated
with the thermal decomposition of coatings, scale, sludge and metals (alloys) in
the work area;
All liquid and solid residues that may contribute to the release of flammable
vapors or gases raising the concentration above the 5 % LEL limit shall be
removed;
Power source to equipment when arc welding shall be isolated for crew breaks
or at end of shift. All electrodes must be removed from holders and placed so
that accidental arcing or contact cannot occur;
Torch and cylinder valves shall be de-pressurised and the whole torch and
assembly hose removed when oxy acetylene cutting, heating or welding is
suspended for breaks or at end of shift;
Compressed gas cylinders or associated manifolds, other than those used for
breathing apparatus, shall not be taken inside the confined space.

4.7.7 Signage

If permanent danger signs is not posted, the Entry Supervisor shall ensure that an
appropriate danger sign has been located near all entrances into a confined space
prior to (and during) personnel entry, and throughout confined space operations while
normally closed entrances are open.

The wording for the Danger signs is:

AUTHORISED PERSONNEL ONLY

A single point of entry must be designated and all other entrances, such as manholes
flanges, shall be barricaded to prevent physical entry of persons and Danger signage
placed identifying the area as a confined space and no entry is permitted.


4.7.8 Ventilation

If welding is required in a Confined Space, some form of mechanical ventilation may
be required that can provide an adequate volumetric rate of fresh air in the vicinity of
the worker(s). Refer to Australian Standards 2865 (Working in a confined space),
1674.1 (Safety in welding and allied processes, Part 1: Fire precautions), and 1674.2
(Safety in welding and allied processes, Part 2: Electrical).

The ventilation power supply should be tagged with an Information Tag to prevent
accidental disconnection.

4.7.9 Training

The Induction training provided to each employee upon initial assignment to project
site shall include sufficient instruction to prevent unauthorized entry into a confined
space.

Employees prior to initial assignment as an Entrant or Entry Supervisor shall
successfully complete a site approved formal classroom training session. Training
content shall include the:

Site-specific requirements for Confined Space Entry; and the;
General duties of Entrants, Controllers and Entry Supervisors.

All personnel required to conduct Authorised Gas Testing shall be required to
complete a site approved formal classroom training session. Training content shall

4.8 RECORDS

All documents relating to issuing and conduct of work associated with a Permit shall
be recorded. Upon completion of the work, all documents shall be collected and filed
by the Site Project Safety Manager.

Completed Permit pads shall be returned to the Safety Section for archiving
purposes, new pads will be issued in exchange for the old ones.
