Education Records Policy (FERPA) 2013-14

The Family Educational Rights and Privacy Act (FERPA) and state law afford students certain rights regarding their
education records. Summarized, they are:

The right to inspect and review the students records. The student may request to review his/her records by submitting a written
request to the registrar or other school official having custody of such records;

The right to seek amendment of the students records that the student believes are inaccurate, misleading, or otherwise in violation
of the students privacy rights. Requests for amendment of records must be in writing and describe the specific portions of specific
record(s) the student wishes to have amended, text or instructions as to the change desired, and reasons why the change is justified;

The right to consent to disclosure of personally identifiable information contained in the students education records, except for
when consent is not required by FERPA. FERPA does not require a students consent when disclosure is to other school officials with
legitimate educational interests. A school official is a person employed by the university in an administrative, supervisory, academic
or research, or support staff position; a person or company with whom the university has contracted or appointed as its agent; or a
student serving on an official committee or assisting another school official in performing the officials tasks. A school official has a
legitimate educational interest if the official needs to review an education record in order to fulfill his/her professional responsibilities.

FERPA also allows universities to disclose a students directory information without consent, but the student may request that
his/her directory information not be released. If the student wishes to make such a request, s/he must do so according to the
procedures outlined in Directory Information Restriction.

The right to file a complaint with the Department of Education, Family Compliance Office, concerning alleged failures by the
university to comply with the requirements of FERPA.


Consent to Release Education Records
Disclosure of Education Record Information

The University of Oregon shall obtain written consent from students before disclosing any personally identifiable
information from their education record. Such written consent for disclosure must: (a) specify the record(s) to be
released; (b) state the purpose of the disclosure; (c) identify the party or class of parties to whom disclosure may be made;
and (d) be signed and dated by the student. All such consents shall be maintained in the education record of the student.

In accordance with state and federal laws, I hereby consent to the release of my University of Oregon educational records
as stated below:
Specific Records to be released:
Academic records (including transcripts, course grades, cumulative grade point average, class rank, etc.)
Financial aid records (including student account records, etc.)
Miscellaneous records (including class schedules or attendance, tutoring session attendance, complimentary
admissions status, parking tickets, etc.)
Other: Disability testing results, student conduct records, drug test results and any other documentation that may
pertain to athletic eligibility determinations

Purpose for the release: To keep athletic department academic staff, parents & guardians appraised of my academic
progress, standing and eligibility; release of class schedules to bookstore to facilitate distribution of course-related
textbooks.

transparent and somehow a toady for the athletic department. I

It's like when a TV network runs a Wednesday night dance
contest, then reports on the results on its Thursday morning
show, then comments on the response to the morning show on
its Thursday night newscast, only to do an in-depth story of the
dancers on its evening news magazine. The network creates its
own news where there is none. Indeed, I fully expect that this
very email will set off a flurry of conversation and debate about
.... what? Nothing.
I would like to point out, however, that this email exchange -
which I am sure is only gathering steam - is a perfect example of
how the currently composed IAC is disfunctional. Its members
find conspiracy and hidden agendas where there are none. This
type of exchange and how it is used to imply bad behavior is one
of the main reasons the FAR has refused to attend meetings, an
elected student representative has resigned, last year's chair tried
to resign, and the athletic department doesn't trust committee
members enough to seek our advice on sensitive topics.
We have made ourselves impotent by confusing non-issues with
real ones and by treating them all in an equally urgent and self-
aggrandizing manner. When everything is a crisis or a
conspiracy, nothing is. It's all just noise - and it's easy to ignore,
even in those instances when there is a real issue.
Let's be clear, I never met or did anything alone. At least five or
six of you were there with me at every meeting, so there's
nothing hidden that would somehow be discovered if there were
minutes. In fact, I think Bill's attendance has been close to
100%. In addition, no one on the committee has ever voted or
made a meaningful decision that related to anything more than
our internal processes.
So, nothing hidden. No secrets. No agenda. No issue or
controversy. No big deal.
This just isn't worth the time or ink we've already devoted to it -
including the time it took me to write, and all of you to read, this
rather too lengthy email.
When I finish my report next week, I will circulate it to the
committee. Hopefully, that will answer any questions you might
have. Until then, I do not intend to be sucked into further back-
and-forth on this issue.

Actually, I was hoping the 3/12 Senate meeting would focus on what to do
about the problematic Jaqua Center student-athlete support issues, which the
IAC, under your leadership, identified this fall, but then did not follow up on.
That is why I filed a Senate motion to address these issues on 2/12, long
before the IAC meeting that you are talking about below. The link to the

Let's be clear, I never met or did anything alone. At least five or
six of you were there with me at every meeting, so there's
nothing hidden that would somehow be discovered if there were
minutes. In fact, I think Bill's attendance has been close to
100%. In addition, no one on the committee has ever voted or
made a meaningful decision that related to anything more than
our internal processes.
So, nothing hidden. No secrets. No agenda. No issue or
controversy. No big deal.
This just isn't worth the time or ink we've already devoted to it -
including the time it took me to write, and all of you to read, this
rather too lengthy email.
When I finish my report next week, I will circulate it to the
committee. Hopefully, that will answer any questions you might
have. Until then, I do not intend to be sucked into further back-
and-forth on this issue.

The University of Oregons new governance structure, enacted by the state
legislature, becomes effective on July 1, 2014. At that time, the UO will no
longer be a part of the Oregon University System (OUS), and will instead be
an independent public university with its own governing board, the Board of
Trustees of the University of Oregon.
As part of the orderly transition, it is advisable to convert all of the existing UO
administrative rules (OARs) to UO policies before July 1. If we do not do this
conversion, these OARs remain in place, requiring legislative action to change.
Therefore, I will adopt all these OARs precisely as written to UO policies. That
action will give our campus community and the board the flexibility and the
time to review carefully each of the policies. To that end, I intend to establish a
policy review advisory group to conduct a deliberate and thorough review all of
these policies and recommend whether any need to be updated, modified,
rescinded, or remain unchanged.
These OARs contain 179 rules in 30 broad categories including rule making,
parking, students records, grievances, and fund raising. The OARs can be
found here. The proposed UO polices that will be adopted verbatim are here.
Public comment on this plan to convert these OARs to UO policies may be
provided in person on June 17 at 3:00 p.m. in 150 Columbia Hall; or in writing
either to the Office of the President, 1226 University of Oregon, Eugene, OR
97403-1226 or via e-mail, by noon on June 18. Oregon State University and
Portland State University are taking similar actions as they transition to having
their own governing boards.
This is the first of several policy actions that are necessary prior to the board
assuming governance of the UO on July 1. The UO will provide notification
about all policy actions on the Board of Trustees website and on AroundtheO.

Basketball is a competitive game which by its very nature, may lead to adversarial relationships
between opponents. However, the relationship should not foster a negative interaction. Taunting,
baiting, ridiculing, finger pointing, trash talking or inappropriate gestures, (obscene,
provoking, and intimidating) are forms of unsportsmanlike behavior. Individuals should employ
a behavior that respects the skill level of an opponent and should not create an atmosphere that
negatively influences the attitude of the game.

The mens basketball committee reiterates that the best mechanism during the championship for
the continued improvement of this behavior is at the institutional level. Therefore, the committee
requests that you review your conduct expectations with your coach, which includes
communications with student-athletes as well as game officials and opponents.

The mens basketball championship is the highest profile event conducted by the NCAA. The Mens
College Basketball Officiating, LLC (MCBO) has engaged and welcomed feedback from multiple
stakeholders regarding sportsmanship. There has been additional discussion on general sideline
decorum between coaches, student-athletes and institutional personnel, typically regarding
unacceptable language or other action in the team area.

When these activities are witnessed by fans or those watching a broadcast, such actions also damage
the game -- a concern to all that support the principles of sportsmanship. All stakeholders must agree
that behavior exhibited by coaches, student-athletes and institutional personnel at the championship
should portray the highest ideals of higher education and intercollegiate athletics.

The mens basketball committee will monitor decorum throughout the championship. With your
leadership, this years championship can demonstrate your personal, as well as our collective
commitment, to assuring that this event will continue to be the best possible experience for student-
athletes and fans alike.

Finally, the committee would like to draw your attention to the following matters:

1. Uniform Rule. You are reminded that all uniforms must comply with the uniform rules
established in the NCAA Basketball 2013-14 and 2014-15 Mens Rule Book. [Rule 1,
Section 22] The uniforms must be readable and visible to both the officials and fans. As such, we
need to see a photo of your uniforms in advance of the tournament. Specifically, we need a color
photo of the uniforms (both home and away and front and back- this includes the jersey
top and shorts) that you will be bringing and using during the tournament.

These photos should be uploaded to the following FTP server at http://files.ncaa.org and should
be done as soon as possible, but no later than 5 p.m. (Eastern time) on Monday, March 17th.

Please note that institutions may also be asked to bring their uniforms to the pre-tournament

Dear Campus Community,
Everyone at the University of Oregon has the right to be free from sexual
violence and harassment, and together we share the responsibility to provide a
safe environment for teaching, learning, and research on campus.
Sexual violence and harassment in any form must not be tolerated. It is our
shared responsibility to foster and demand a culture in which the prevention of
sexual violence and harassment is paramount, reporting expectations and
processes are clear, excellent support services are readily available and easily
accessible, and those who violate our policies receive appropriate discipline.
As we begin spring term and Sexual Violence Prevention Month, I write to
reaffirm my commitment to ensuring a safe campus for all. Anything less is
contrary to the values of our institution, and to the commitment we make to
our students and each other.
The UOs efforts to create a campus free of sexual violence and harassment
begin with prevention. The university has in place a robust set of initiatives
designed to inform and educate students, faculty, and staff about how they can
help prevent sexual violence and harassment and promote bystander
intervention.
These initiatives include a new website, scheduled to launch this month, that
provides comprehensive information about preventing and reporting sexual
violence and harassment, as well as support resources for students who have
experienced sexual violence and harassment. The Dean of Students Office is
hiring an additional staff member to oversee our prevention efforts and
coordinate student-led prevention initiatives. In addition, there are many
groups and individuals committed to addressing this issue, including the
Alliance for Sexual Assault Prevention; the student-driven Sexual Wellness
Advocacy Team (SWAT); the Student Affairs Sexual Violence Support,
Prevention, and Education staff; and a new student advisory team that will
promote prevention efforts.
Many programs seek to educate the campus community about preventing
sexual violence and harassment, including:
All new students under the age of 21 entering the university are required
to take the online course module, AlcoholEdu and a sexual assault
course module, Haven.
The Sexual Wellness Advocacy Team (SWAT) conducts an educational
performance during IntroDUCKtion, It Cant be Rape, and provides
numerous prevention presentations and workshops throughout the year
on campus.
SafeRide prevention shuttle offers all UO students, faculty, and staff a
comfortable and safe means to travel at night.
At the start of each school year the Red Zone Campaign, sponsored by
the ASUO Womens Center and Dean of Students Sexual Violence
Prevention and Education program, educates students about sexual
violence.
The University Counseling and Testing Center provides confidential
counseling sessions and offers resources on its website.
A sexual violence prevention internship partnership between the Sexual
Violence Prevention and Education program and University Housing
launched in the fall of 2013 to address prevention in all housing
complexes.
The Dean of Students Office and International Affairs are finalizing a
video that will address interpersonal violence with our international
student population.
The ASUO Womens Center and community non-profit, Sexual Assault
Support Services (SASS), coordinate the annual Take Back the Night
rally each April.
The ASUO Mens Center addresses sexual violence on campus through
programs including sexual communication workshops, presentations to
classes about mens role in preventing sexual violence, and the Break
the Cycle event in April.
All university employees are required to complete an online training
program on Workplace Harassment Prevention.
When sexual violence does occur, it is critical that survivors have access to
reporting options and be connected with support services. Students who have
experienced sexual violence or harassment have confidential and non-
confidential options for reporting the incident. In the past year, we have added
three additional staff positions to facilitate this supportone each in the UO
Police Department, the Office of the Dean of Students, and the Office of
Affirmative Action. The University Counseling and Testing Center has created
a new Interpersonal Violence Response Team comprising five senior clinical
staff members and at least one doctoral intern, all of whom have extensive
training or experience with trauma treatment. Additionally, the University
Health Center provides confidential physical and emotional care following
incidents of sexual assault, dating/domestic violence, and stalking.
Reported incidents of sexual violence and harassment are taken very
seriously and are thoroughly investigated by the Office of Affirmative Action
and through the student conduct code process. We also work closely with law
enforcement when appropriate.
The Office of the Dean of Students has prepared a comprehensive resource
guide for the UO community to help students who have experienced sexual
violence or harassment, including domestic/dating violence, sexual assault, or
stalking. This guide outlines support services that are available from the
university, in the community, and through law enforcement, and explains how
to access each of these options. The new 24/7 Sexual Violence Response and
Support Services hotline541-346-SAFE (7233)will be activated early next
week.
If you have comments, suggestions, or questions about the University of
Oregons sexual violence and harassment prevention and support efforts
please contact Sheryl Eyster, associate dean of students. For more
information about employees reporting responsibility and students reporting
options when faculty or staff is involved please contact Penny Daugherty,

Presidents and Chancellors:
Enclosed with this e-mail is the link to the NCAA Financial Dashboard for your institution, comprising five
years of data through FY2013. This is a review and planning tool designed to assist presidents and
chancellors with financial decision-making regarding their intercollegiate athletics programs. The
indicators provide a research-based approach to financial reporting through uniform and common
definitions, as well as the opportunity for comparisons against peer institutions of your choosing. The
indicators are calculated from the data submitted to the NCAA in January of each year. Therefore, it is
vitally important that you review the accuracy of the data supplied to the NCAA annually so the
indicators can be correctly calculated for comparison purposes.
Changes to the 2013 Dashboard:
With the elimination of the Capital Expenditures Survey, any indicators based on this information will no
longer be available.
The link to your institution's dashboard is below:

Safeguards have been designed within the tool to ensure institutional confidentiality. Please note there
is no means to access the dashboards from the NCAA web site. Therefore, please save this e-mail or
the link as a favorite in your browser for future access. Your chief financial officer, athletics director,
and faculty athletic representative listed for your institution in the Financial Reporting System are also
receiving this Email and link. If you feel there are others on your campus that you would like to have
access to the data please forward this email on to them as well.
NCAA Revenue Expense Reporting Focus Group:
In February 2013, the NCAA assembled a focus group of institutional financial officers, athletics business
officers and athletic directors to perform a review of the NCAA Revenue Expense Reporting to identify
ways to provide more meaningful and consistent financial reporting to university presidents. There
were many areas for improvements and efficiencies identified. The focus group review will be completed
in June 2014. The findings will be published before the end of 2014 and the changes will be
implemented in 2015-2016 fiscal year.

Dear Campus Community,
The report of sexual violence affecting our community is of deep concern to
me, as it is to all of us. The University of Oregon takes such reports extremely
seriously. As a residential campus, a close community of students, faculty, and
staff, supporting our students and protecting their safety is our highest priority.
I am deeply troubled by the information contained in the police report released
yesterday by the Eugene Police Department. The university has rigorous
internal conduct processes that we follow when we receive a report such as
this, as well as legal processes and a moral commitment to our students. We
share a responsibility to provide a safe learning environment for our students,
a responsibility I take very seriously.
