STEP 3: Attend an OIE Application Session at OIE
Call 412-268-5231 and make an appointment to be scheduled for an OPT session 2-3 months prior to theanticipated completion of your program. You will need to bring your passport, I-20, I-94, and thecompleted department recommendation form with you. Please complete attached I-765, leaving blank anyfields about which you have questions. This form can also be found on-line. It is only necessary to printpages 12 & 13 of the online form at: http://uscis.gov/graphics/formsfee/forms/files/i-765.pdf. Note: thecode for OPT in item # 16 is (C)(3)(i).
STEP 4: Complete the USCIS Application Materials
Once you have received the new SEVIS I-20 with the OPT recommendation from the Foreign StudentAdvisor, you will need to complete the application materials, make photocopies of all the paperwork foryour files, and send the entire application to the USCIS to complete the authorization procedure. DO NOTUSE THE e-FILING PROCEDURE—it slows the application process.HERE IS WHAT YOU MUST SEND TO USCIS:
A $340 check or money order payable to “Department of Homeland Security.”
Original I-765 form completed by you with your original signature – 7/30/2007 version (see bottom right cornerof form). The code for OPT in item #16 is (C)(3)(i). Note that the address you use must be valid for at least 3months from the time you send the application. The U.S. Postal Service will not forward the EAD card even if you leave a forwarding address. If you chose to use OIE as your mailing address, put the address as:

Photocopy (NOT original) of new SEVIS I-20 with the OPT recommendation from OIE Advisor

You must return to OIE to collect your new I-20 with the OPT recommendation before you can proceed withthis application.
Photocopies (front and back) of all previous “old style” and SEVIS I-20 documents dating back to beginning of F-1 status.
Photocopy of I-94 (front and back).
Photocopy of the most recent I.D. page(s) of your passport (these are the page(s) which contain the picture,number, and expiration date).
Photocopy of the most recent U.S. visa stamp (in a page of your passport).
Two “passport” photos. NOTE: this is a change from the previous photo requirements. More details about canbe found at: www.travel.state.gov/passport/pptphotos/index.html. In pencil, print your name and write youradmission number (I-94#) on the back of each photo. Copy of a previously issued EAD card if available.
NOTE: If you use the OIE address and want OIE to mail the card to you later, you must pay for card shipping by check,payable to Carnegie Mellon University, in advance. OIE uses UPS for express service. We will not use prepaid shippingenvelopes from other delivery companies.
STEP 5: Mail the Application Pack to USCIS
When you have completed the entire application, make a complete set of photocopies for your files, organizethe materials as neatly as possible and send all of the above items to the appropriate USCIS Service Centerhaving jurisdiction over the address you have used on the I-765. For those using a return address inPennsylvania on the I-765, the Service Center address is:USCIS Vermont Service Center75 Lower Welden St.St. Albans, VT 05479-0001We recommend that you send the application via an express mail carrier or via U.S. registered mail “return-receipt requested” so that you have proof of delivery. Your permission to begin Optional Practical Training isnot finalized until you have received an “Employment Authorization Document (EAD)” from the USCIS.
STEP 6:
Wait For the EAD
Within one month of sending the application to the USCIS, you should get a standard receipt notice stating thatUSCIS has received the application and that it will take 80 days to process (this is not an actual estimate; it isthe time stated in every letter). If you do not get this letter within 6 weeks contact an advisor for moreinformation. The “Receipt Number” in the top left corner of this receipt notice can be used to check the status
