Glioblastoma Multiforme
(GBM) is the most common and mostaggressive type of primary brain tumor in humans, involving glial cells andaccounting for 52% of all parenchymal brain tumor cases and 20% of allintracranial tumors. Despite being the most prevalent form of primary braintumor, GBMs occur in only 2–3 cases per 100,000 people in Europe andNorth America. According to the WHO classification of the tumors of thecentral nervous system, the standard name for this brain tumor is"glioblastoma"; it presents two variants: giant cell glioblastoma andgliosarcoma. Glioblastomas are also an important brain tumor of the canine,and research is ongoing to use this as a model for developing treatments inhumans.Even with current best efforts at treatment, most patients who arediagnosed with Glioblastoma eventually die from it. Treatment can involvechemotherapy, radiation and surgery. Median survival with standard-of-careradiation and chemotherapy with temozolomide is 15 months. Median survivalwithout treatment is 4 1/2 months. Surgery is controversial, since norandomized controlled trials have been done.Symptoms -General symptoms of

are the same as for brain tumors. Specific symptoms will depend on the size and location of theglioblastoma multiforme. Although common symptoms of the disease includeseizure, nausea and vomiting, headache, and hemiparesis, the single mostprevalent symptom is a progressive memory, personality, or neurologicaldeficit due to temporal and frontal lobe involvement. The kind of symptomsproduced depends highly on the location of the tumor, more so than on its
 
pathological properties. The tumor can start producing symptoms quickly, butoccasionally is an asymptomatic condition until it reaches an enormous size.A neurologic evaluation should be done if a patient has slowlyincreasing signs of mental dysfunction, new seizures, persistent headachesor evidence that there is pressure inside the skull, such as vomiting or swelling or protrusion of the blind spot at the back of the eye. When viewedwith MRI, glioblastomas often appear as ring-enhancing lesions. Theappearance is not specific, however, as other lesions such as abscess,metastasis, tumefactive multiple sclerosis, and other entities may have asimilar appearance. Definitive diagnosis of a suspected GBM on CT or MRIrequires a stereotactic biopsy or a craniotomy with tumor resection.Depending on the patient's symptoms, specialized tests may be done,including tests of the field of vision, the sharpness of vision and hearing. If theresults of other tests are not conclusive, an examination of the fluid thatsurrounds the brain and spinal cord may be done, although it is usuallyunnecessary.Treatment -It is very difficult to treat glioblastoma due to several complicating factors:
