Dear Ms. Richardson,

This is a request under the North Carolina Public Records Law (General Statutes, Chapter 132) for
information pertaining to Bradley Mark Walker, a member of the Greensboro War Memorial
Commission. Specifically, I am requesting the following:

Appointment & Salary Details

1. Copies of all Board / Commission interest form and any other application materials
(including questionnaires and resumes) submitted by Walker for appointment to any City
Board or Commission since January 1, 2008.

2. The official date of Walkers appointment to the War Memorial Commission and, if
applicable, the official date of his departure from the Commission.

3. If Walker is no longer a member of the Commission, please provide all correspondence
between Walker and City Staff regarding his departure from the Commission, including any
letters of resignation submitted by Walker.

4. Details of any compensation, benefits (such as health insurance), allowances (such as cellular
phone or vehicle allowances), or any other payments Walker received from the City.

Travel Reimbursements & Other Expenses

5. Details of all reimbursements, including travel reimbursements, from the City to Walker,
from January 1, 2011 to present. Please include all available backup documentation on these
reimbursements, including copies of invoices and receipts.

6. Details of all travel expenses (whether reimbursed or paid directly by the City) incurred for
Walkers travel from January 1, 2011 to present. Please include all available backup
documentation on these expenses, including copies of invoices and receipts.

7. Details of all charges incurred by Walker on any City-issued credit card from January 1, 2011
to present. Please include all available backup documentation on these expenses, including
credit card statements, copies of invoices, and receipts.

8. The value of any computer, laptop or tablet issued to Walker from January 1, 2012 to
present. Also, if the City has issued Walker one of these devices during this time period,
please provide the date range when Walker had this device, and please also provide details of
the devices brand and model.

Meeting Minutes & Annual Budgets

9. Copies of all War Memorial Commission agendas, agenda packets/staff reports/staff
memoranda, and meeting minutes during the entire period of Walkers service of the
Commission. If possible, please provide these in an electronic format.

10. Copies of all approved annual budgets for the Citys War Memorial Coliseum Complex
Fund from January 1, 2011 to present.
