II.
PRELIMNARY
PROCESS:
o
The Assistant
Chief
on
B
Shift or
designee
will
provide
a
list
of
current hydrant
listings
with
the correct hydrant
numbers.
o
The Assistant
Chiefor
designee
will
work
together
as a
team
to
assign
hydrants
to
the
different
shifts
and apparatus, assigning
them
by
stations and
corresponding
response zones.
o
The
hydrant assignment
list
will
be
submitted
to
the Assistant
Chief
for final
approval.
o
The Assistant
Chief
on
B
Shift
will
notify
the Water
Division
of
the
date
that
testing
will
begin
and
will
be
responsible
for notifying
the
Water
Division
of
anychanges
and
sending
the
PIO information
for
a
Press
Release.
.
The
list
provided
by
B
Shift
Assistant
Chiefwill
be
typed up
and
a database
created
to track the
assigned
hydrants, shifts,
and assigned apparatus.
o
This
will
become
the
ofiicial
list
that
will
be
printed out
and
used every
time
hydrants
are
tested.
The
list
will
only
be changed
if
new hydrants
have beenadded
or
if
hydrants
have been
removed.
.
A
copy
ofhydrant
assignments
will
be
disseminated to the
Chiefof
Communications
and
the Water
Division
in the
case
that
follow
up on
a
specifichydrant
needs
to
be conducted-
III.
FORMS:
r
There
are
four
forms that
will
be
used
for
checking hydrants, the hydrant
work
order
fornr,
hydrant
flow
correction form,
and
the hydrant testing
form.
Any
updates
or
revisions
will
be
provided
by
B
Shift.
o
The hydraat testing
form
is
to
be
completed every
time
tbat
a
hydrant
is
physically
checked
by
a
firefighter.
e
The hydrant
work
order
form
is
to
be
completed by
the
firefighter
conducting the
test
ifthe
hydrant
is
inoperable
or
is
not
functioning
properly.
o
The hydrant
work
order
form
must
have
a
brief
description
ofthe
problem
which
contains
numerical
numbers,
which
speci$
the performance
level ofeach hydrant
checked.

The
Hydrant
Flow
Correction
Form
is
for
hydrants
with
the
same
problems.
o
The
New
Hydrants
&
Hydrants
Not
Listed Form.
This form
is
to
be
used
only
to
list
New
Hydrants
and
Hydrants not
listed
on
Hydrant Flow
Form.
o
The numbers
denote
the level
ofproficiency
or
rate
of
performance
ofthat
particular hydrant.
l:poor,
2=fair,3=gcr,d,
and
4=excellent. The letters
A
L,
M
and
O
stand
for
American
Darling,
Ludlow,
and
Mueller which
are
the mostcommon hydrants in
this
are,
with
O
signifying
other.
o
The hydrant
numbers
that
will
be
used
on
all
paperwork
will
be
numbers specifiedby
B
Shift
or
LCFD
Record Keeping
Division.
o
Each hydrant
will
be assigned
a
four
digit
identification
number
that
will
be used
on
all
corresponding paperwork
for
that specific hydrant.Revised
