1. YOU ARE NOTIFIED that the plaintiff(s) demand(s) from you the amount of $4,601.95 plus interest at the
contract or maximum lawful rate and costs, based on a deficiency which remains after the sale of certain
secured personal property originally owned by WELLS FARGO BANK . Said account is now owned by the
plaintiff. Interest at the rate of 18.93% from June 30, 2009 or the date of default per contract to the date of
suit is also requested in sum of $4,451.21 and costs. Total amount requested $9,053.16.
2. Judgment may be entered against you unless you file an Appearance and Answer within 20 days of the
service of the Original Notice upon you. Judgment may include the amount requested plus interest and
court costs. You must file an Appearance and Answer with the clerk of the district court in the above
county, located at: SAC County Clerk of Court, 100 NW STATE ST. SUITE 12
SAC CITY, IA 50583 .
3. If your Appearance and Answer is filed within 20 days and you deny the claim, the clerk will notify you of the time and
place for the hearing on this matter.
4. If you file an Appearance and Answer, you must mail a copy of the form to Plaintiff(s) or to the attorney for
