Gives factual information, analyzes and describes the probable cause of the October 4, 1992 aircraft accident at Amsterdam, Bijlmermeer.
Shortly after take-off from Schiphol Airport the no.3 engine of the El Al Boeing 747 aircraft separated from the wing, clipping the no.4 engine which also separated.
During the emergency approach back to Schiphol the aircraft lost control and crashed into an apartment building. It was concluded that te engine separation was caused by failure of the inboard midspar fuse-pin.
