OBSERVATIONS:
 Dish machine observed with highest temperature of 147F after 3 cycles. Machine was actively in use during inspection. Use sink to sanitize dishes until repaired. REPEAT VIOLATION

Food-contact surfaces and multiservice utensils shall be effectively washed, rinsed, and sanitized to remove or completely loosen soils by use of manual or mechanical methods. The temperature of the washing solution shall be maintained at least 100F or at the temperature specified by the manufacturer's label. Manual sanitation shall be accomplished during the final rinse by contact with solution containing 100 ppm available chlorine or 200 ppm quaternary ammonium. Mechanical sanitization shall be accomplished in the final rinse by achieving a utensil surface temperature of 160F or by contact with solution containing 50 ppm available chlorine. 23VERMIN AND ANIMAL CONTAMINATION
OBSERVATIONS:
 ~20 live cockroaches observed at valve below long stretch of dirty dish table in warewashing area.~20 live cockroaches observed around/behind thin plumbing that runs below dirty dish table from machine to end of table. 2 live cockroaches observed at temperature dial of unused steam table at front service line.1 live cockroach observed below dry goods table at end of cook's line. Cockroaches appear to be German, multiple life stages observed below dish table. Facility is closed & health permit suspended until cockroach infestation is abated. Contact EMD for reinspection after pest control all serviced to eliminate the infestation. Remove all unused equipment that provides hiding areas for pests, clean entire facility including hard to reach places & dishes/utensils as they have been subject to pests. Seal all areas of walls/ceiling where pests may enter facility.Perform self inspection with a light throughout facility to ensure no live activity of cockroaches is observed prior to calling for reinspection. 41PLUMBING FIXTURES AND BACKFLOW PREVENTION
OBSERVATIONS:
 Leak observed at left side faucet of 3-compartment sink. Repair within 7 days.Leak observed at left side of steam bath in kitchen. Repair within 7 days. REPEAT VIOLATIONIce machine is draining to floor that is not sloped to drain. Provide plumbing to direct drain to floor sink/drain with pipe ending 1 inch above the drain.

Steam tables, ice machines, ice bins, food preparation sinks, display cases, walk-in refrigeration units, and other similar equipment that discharge liquid waste shall drain into a floor sink with an air gap or other approved type of receptor. Plumbing and plumbing fixtures shall be maintained so as to prevent contamination, shall be kept clean, fully operative, and in good repair. The  potable water supply shall be protected with a backflow or back siphonage protection device. Food facilities shall be fully enclosed in a building consisting of permanent floors, walls, and an overhead structure that meets minimum standards. The walls and ceiling shall be smooth, durable, nonabsorbent, and easily cleanable. Flooring and base coving shall be smooth, durable, and made of approved nonabsorbent material that is easily cleanable. Floor surfaces shall be coved at the juncture of the floor and wall with a 3/8 inch minimum radius and shall extend up the wall at least 4 inches.
