23VERMIN AND ANIMAL CONTAMINATION
OBSERVATIONS:
 3 live German cockroaches observed at front service line at temperature dial of steam table; 1 adult, 1 adult with egg case & 1 juvenile. This location observed as a heavy fecal focal point today & 12/18/14. Employee attempted to kill roaches. Appears to have caught both adults.1 live German juvenile cockroach observed at electrical components inside walk in used as dry storage. Killed by employee. Fecal focal point observed at adjacent circular vent as well. When electrical box was opened, 2 dead German cockroaches observed inside.MINOR VIOLATIONS~10 dead German cockroaches observed throughout dishwasher area, various stages of life.1 dead German cockroach observed outside of walk in cooler near main kitchen.1 dead German cockroach observed bel0w table between walk -in & dry storage walk in.DUE TO LIVE COCKROACH ACTIVITY FACILITY IS REQUIRED TO REMAIN CLOSED. NO FOOD MAY BE HANDLED OR DISTRIBUTED UNTIL REINSPECTION IS CONDUCTED BY EMD TO ENSURE INFESTATION HAS BEEN ABATED.-Notify pest control of locations where live activity was observed for service. Clean & sanitize all effected areas.-Remove all dead bodies & sanitize effected areas.-Continue to perform self-inspection of facility & call EMD for reinspection at 875-8440 when no live activity is observed. -Continue to clean all food & grease residues from facility (ex. fire suppression at griddle, underside of fryer & under counter elements at front service line).-Location of activity may change as treatment is applied. Entire facility must be observed free of any live activity at EMD reinspection for permit to be reinstated. -Move all boxes, equipment away from wall for thorough inspection by yourself, pest control & inspector at reinspection. Particularly below dish washer tables & dry storage walk in.-Eliminate all areas where pest can hide or enter (see report dated 12/18/14- many of the noted areas remain).
