Fundraiser Registration Form.
All Jeb 2016 contributions must be received by our office by close of business
Tuesday, June 30, 2015, in order to qualify. Timely receipt is critical for our June
30th reporting deadline.
Only primary election contributions will count toward your 27 in FIFTEEN goal.
All participants are requested to track their individual contributions and remind
all donors to include the participants name or tracker number on all
contributions to ensure proper attribution.
Upon meeting your $27,000 goal, our team will email further details on 27 in
FIFTEEN events and benefits.

Contributions to Jeb 2016, Inc. are not tax deductible as charitable contributions for federal income tax purposes.
Federal law requires us to use our best efforts to collect and report the name, mailing address, occupation and
name of employer of individuals whose contributions aggregate in excess of $200 per election cycle.
Contributions from corporations, foreign nationals (non-green card holders), federal government contractors and
minors under the age of 16 are prohibited.
