    This is just a simple chart I created to help me organize my clinical day with my patient. It helped me be able to look ahead & plan for interventions that needed to be done, medications that needed to be given, & keep track of any changes or new results I needed to focus on. I liked having everything in one place. We also use an SBAR during clinical, but a lot of the nurses on the floors like using similar charts like this. It depends on the hospital & floor policies.
