Philippine Consulate General, Jeddah, KSA PASSPORT RENEWAL, ISSUANCE & REPLACEMENT REQUIREMENTS (as of 01 January 2010) Requirements for Renewal of Philippine Passport:

• Personal Appearance
• Duly-accomplished Passport Application Form
• Thre (3) photos 1.77\"x1.37\"(4.5 cm.x3.5cm.) with royal blue background. Picture must be recent, taken within a period of 6 months.
• Must present the original old Passport with photocopy of pages 1 and 33.
• Fee: SR200.00

Requirements for the Issuance of a New Passport for Children Born in the Kingdom:

• Duly accomplished Passport Application Form
• Thre (3) photos 1.77\"x1.37\"(4.5 cm.x3.5cm.) with royal blue background. Picture must be recent, taken within a period of 6 months.
• Original Birth Certificate duly authenticated by the Ministry of Foreign Affairs, KSA (translated into English and photocopies of the Arabic Birth Certificate and the English translation.
• Marriage Contract of parents duly authenticated by the Department of Foreign Affairs in Manila if solemnized in the Philippines or by the Saudi Ministry of Foreign Affairs, if solemnized in Saudi Arabia (with English translation)
• Photocopy of parents' passports (pages 1 and 33 or 44), two (2) copies
• Duly accomplished Report of Birth Form, one (1) copy
• Duly accomplished Birth Description Form, one (1) copy
• Fe: SR300.00

Replacement of Lost Passport:

• Duly accomplished Passport Application Form
• Thre (3) photos 1.77"x1.37" (4.5 cm.x3.5cm.) with royal blue background. Picture must be recent, taken within a period of 6 months.
• Photocopy of lost Philippine passport.
• 15-day waiting period to allow issuance verification from Manila or the appropriate place of issuance if not issued by PCG Jeddah
• Police Report on the loss of the passport and its translation into English.
• Photocopy of the newspaper publication reporting the loss of the passport and its translation into English.
• Two (2) copies of the Affidavit of Loss.
• Fe: SR460.00 (if lost passport was issued in Jeddah)
• Fe: SR540.00 (if lost passport was issued in Manila) Duration: Thirty (30) calendar days from date of application.
