 TRAFFIC SAFETY SCHOOL INSTRUCTIONS — Complete this side only if you chose Box B.
CLASS LOCATIONS: Classes are offered Monday
through Saturday, 8:00 am to 12:00 noon, 1:00 pm to
5:00 pm and 6:00 pm to 10:00 pm (evening classes do
not meet on Saturday). The 4-hour class is completed
in one session.The 8-hour class is completed on two
consecutive weeknight evenings or one Saturday
session. Classes are offered in English, Spanish and
Polish. Sign language interpretation is available upon
request.

TRAFFIC SAFETY SCHOOL: Your class assignment form
will be mailed to you. Some class locations have limited or
restricted schedules. Class locations are subject to change.
Your request will be honored, if possible, or a change to the closest
possible location will be made. If you are unable to attend your
scheduled class, immediately call Traffic Safety School and
reschedule. To reschedule at no fee, call no later than the date
stamped on your class assignment form. After that date the
rescheduling fee is $20. If you miss your class, you must
reschedule another class and pay $20. It is your responsibility to
notify the Traffic Safety School if you change your mailing
address. The school is not responsible for late, lost and/or
undeliverable mail.
IF YOU COMPLETE CLASS YOU WILL NOT NEED TO
APPEAR IN COURT: Traffic Safety School will report to the
court when you complete class. No driving conviction will be
reported to the Secretary of State.
